Objective: To know the resilience and mental health status of therural left-behind middle school students in Yuyang county, to analyze theinfluence factors of resilience in left-behind middle school students.Establish a prospective cohort study, to discuss the dynamic variation ruleof resilience and mental health in left-behind middle school students, thussystematically understand the mental state of rural left-behind middleschool students and provide scientific basis for targeted interventionmeasures.Methods: A baseline survey was done by questionnaire in1403middle school students who were selected in3junior and seniormiddle-schools in Yunyang county by multistage stratified cluster randomsampling. Eliminate the students who were with mental illness or hadmental health problems in baseline survey, the remaining1322studentswere chosen to proceed a one year follow-up observation, a total of twofollow-up and the frequency of follow-up was once every six months. To observe the dynamic variation rule of resilience and mental health inleft-behind middle school students. The self-designed questionnaire,Resilience Youth Development Module scale and Mental Health Testscale were used in both baseline and follow-up investigation.Results: The number of questionnaires for the baseline surveyanalysis was1403, including1049left-behind middle schoolstudents(LBS),352non-left-bhind middle school students(NLBS) and2orphans.1) The detection rates of low resilience in LBS was0.95%, thatof medium resilience was54.15%, and that of high resilience was44.90%.The total average score of the RYDM in LBS was2.96±0.41and therewas no significant difference between LBS and NLBS (P>0.05). Theresult of gender differences showed that male students scoredsignificantly higher than female students in meaningful participation inthe home and self-efficacy (P<0.05), while female students scored higherthan male students in the caring peer relationships and empathy (P<0.05).And the result of grade differences showed that apart from the caring peerrelationships, empathy, and cooperation and communication, junior gradeone students scored notably higher than senior grade one students in allthe other dimensions (P<0.05).2) The detection rate of mental problemsfor LBS was6.01%while that for NLBS was4.55%. The difference ofdetection rate between LBS and NLBS was not significant(P>0.05). Thecorrelation analyses showed that the total score of resilience had a significant negative correlation with the total score of mental health andthe other6sub-factor scores of mental health apart from two factors:learning anxiety and self-accusation tendency.3) The influence factorsassociated with the resilience of LBS included grade, working status ofparents, attitude for parents’ working outside, students’ worry about theirparents working outside and the living situation of students.A total of1322rural middle school students were into the cohortstudy, eventually1101finished the follow-up and the follow-up rate was83.28%.1)The result of the changes in the basic information ofparticipants over time showed that LBS and NLBS had significantdifference in whether have brothers and sisters, living situation ofstudents and subjective economic status in baseline survey, and the twogroups had significant difference in gender and whether have brothersand sisters in the last follow-up investigation.2)The result of the changesin resilience of middle school students over time showed that the averagescore of RYDM in LBS was2.999in baseline survey, was2.926in firstfollow-up investigation and was2.909in second follow-up investigation.The overall level of resilience in LBS showed a trend of gradual decline,but their resilience in general was still in the medium level over time.3)The result of the changes in mental health status of LBS over timeshowed that the average score of MHT in LBS was41.683in baselinesurvey, was40.139in first follow-up investigation and was39.238in second follow-up investigation. The overall mental health situation wasbetter and presented a gradual upward trend, but the LBS who were withmental health problems gradually increased over time.Conclusion: The detection rate of mental health problem of ruralmiddle school students in Yunyang county was low, the resilience level ofmost LBS was better. The resilience of LBS was influenced by manyaspects. Thus, it is necessary that families, schools, and individualconditions should be combined together to promote well adaptation ofLBS.The resilience level of LBS was still kept the medium level overtime, the mental health situation was better and presented a gradualupward trend, but the students who were with mental health problemsgradually increased over time. It suggested that relevant departmentsshould take the targeted intervention measures for the students who werewith mental health problems.
