Objective: To explore the distribution characteristics of TCM syndrome types ofelderly type2diabetes mellitus-related insomnia.Methods: Inclusion criteria and exclusion criteria were established. Then300cases ofpatients over60years old with type2diabetes mellitus and insomnia were collected.The patients’ situations were investigated, including general conditions, type2diabetes mellitus-related history, clinical symptoms, tongue and pulse conditions,body mass index, waist circumference, fasting plasma glucose, glycosylated serumprotein, and glycosylated hemoglobin A1c. Mathematical statistics methods such asfrequency analysis and K-means cluster analysis combined with TCM syndromedifferentiation methods such as syndrome elements differentiation were used toanalyze the distribution characteristics of TCM syndrome types of elderly type2diabetes mellitus-related insomnia. Also, factor analysis was used to explore thecharacteristics of common factors in different TCM syndrome types.Results:1.Clinical symptoms and tongue and pulse conditions: The clinicalsymptoms whose occurrence frequency were higher than60%included insomnia, drymouth, thirst and polydipsia, dizziness, constipation, increased nocturia, lassitude andhypodynamia, and limb numbness. And the tongue and pulse conditions whoseoccurrence frequency were higher than60%included thready pulse and white tonguecoating.2. K-means cluster analysis: When the number of clusters was7, the resultof K-means cluster analysis was best. According to the significance of TCMsyndrome differentiation of each cluster group, the TCM syndromes could bedifferentiated into seven types, including Qi deficiency syndrome, Yin deficiencysyndrome,heat syndrome, deficiency of both Qi and Yin syndrome, deficiency of both kidney-Yin and kidney-Yang syndrome, liver-Yin deficiency syndrome,anddeficiency of kidney essence syndrome. By comparison, the differentiation degree ofthis kind of classification method was better, and the classification result wascomparatively consistent with the understandings in both TCM theories and clinicalstudies.3. Factor analysis: There were different common factors in different clustergroups. And the common factors in Qi deficiency syndrome, Yin deficiencysyndrome,deficiency of both Qi and Yin syndrome, and deficiency of kidney essencesyndrome were obviously different from those in the other TCM syndrome types.Conclusion:1. The TCM syndromes of elderly type2diabetes mellitus-relatedinsomnia can be differentiated into seven types, including Qi deficiency syndrome,Yin deficiency syndrome,heat syndrome, deficiency of both Qi and Yin syndrome,deficiency of both kidney-Yin and kidney-Yang syndrome, liver-Yin deficiencysyndrome,and deficiency of kidney essence syndrome.2. There are differentcommon factors in different TCM syndrome types, and there is great differenceamong the common factors in certain TCM syndrome types.
