Discussion On TCM Syndrome Types And Related Factors Of Type 2 Diabetes Complicated With Insomnia

Purpose:The relationship between TCM syndrome type and age,duration,blood sugar,blood lipid and complications was analyzed through the study of 118 patients with type 2diabetes mellitus combined with insomnia.It provides some clinical and theoretical reference for traditional Chinese medicine treatment of type 2 diabetes mellitus combined with insomnia,and also provides reference for guiding patients to change their lifestyle,improve their quality of life,and slow down complications.Material and method:1 From October 2017 to October 2018,patients who met the diagnosis of type 2 diabetes mellitus were admitted to the Department of Endocrinology of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine.After consultation,they were diagnosed with insomnia and agreed to participate in the researchers.A total of 118 people were selected.2 Collecting patient information,including the patient's sex,age,duration,place of residence,Determination of fasting plasma glucose(FPG),hemoglobin A 1c(HbA1c),triglyceride(TG),total cholesterol(TC),high density lipoprotein(HDL-C),Low density lipoprotein(LDL-C)for selected patients,inquire about and record complications,Refer to the "guiding principles of clinical research of Chinese medicine new drugs(Trial Implementation)"(2002)to determine the type of Chinese medicine syndrome in conjunction with the actual clinical conditions of patients in our hospital endocrine department.3 The general data,laboratory indicators,complications and syndrome differentiation of patients with type 2 diabetes mellitus combined with insomnia were analyzed by SPSS19.0 statistical software.Results:1 Among the 118 patients included,the sex was more common among women(66.1 %);The age is mostly middle-aged(54.2 %);The majority of patients with a duration of more than 10years(37.3 %);The residence is mainly in the city(75.4 %).2 Chinese medicine syndrome type: 118 patients with Chinese medicine syndrome type are divided into Yin deficiency and internal internal heat syndrome,Qi and Yin deficiencysyndrome,Qi and Yin deficiency and blood stasis syndrome,wet and heat trapped spleen and blood stasis syndrome,Yin and Yang deficiency syndrome with blood stasis syndrome,of which Qi and Yin deficiency and blood stasis syndrome The largest number of people,accounting for 42.4 %,The rest are the Yin and Yang deficiency syndrome with blood stasis syndrome,wet and heat trapped spleen and blood stasis syndrome,Qi and Yin deficiency syndrome,Yin deficiency and internal internal heat syndrome.3 Gender,age and duration of disease distribution among each syndrome type: the gender distribution among each syndrome type was not statistically significant(P>0.05),while the age and duration of disease distribution among each syndrome type was statistically significant(P<0.05).Type 2 diabetes mellitus combined with insomnia patients according to age,length of disease,the symptoms of the syndrome are Yin and Yang deficiency syndrome with blood stasis syndrome,Qi and Yin deficiency and blood stasis syndrome,wet and heat trapped spleen and blood stasis syndrome,Qi and Yin deficiency syndrome,Yin deficiency and internal internal heat syndrome.4 The changes of blood glucose and blood lipids in different syndrome types: there was no statistically significant difference in HDL-C between different syndrome types(P>0.05),and there were statistically significant differences in FPG,HbA1 c,TG,TC and LDL-C between different syndrome types(P<0.05).).The fasting plasma glucose and hemoglobin A 1c of Yin and Yang deficiency syndrome with blood stasis syndrome were the highest,and the triglyceride,total cholesterol and low density lipoprotein were the highest in wet and heat trapped spleen and blood stasis syndrome.5 The complications of 118 patients included diabetic nephropathy,diabetic retinopathy,diabetic peripheral neuropathy,diabetic lower extremity vascular disease,and diabetic carotid artery disease.Among them,the incidence of diabetic lower extremity vascular disease was the highest,accounting for 60.2%.6 Complications among the syndrome types: There were statistically significant differences in the number of complications between the syndrome types(P<0.05).The number of complications was the highest in the patients with Yin and Yang deficiency syndrome with blood stasis syndrome.The average number of complications in the syndrome patients was(2.77±1.28).Different syndromes had statistically significant effects on the incidence ofdiabetic peripheral neuropathy,diabetic lower extremity vascular disease,and diabetic carotid artery disease,but had no statistically significant effect on diabetic nephropathy and diabetic retinopathy.The most common complication of Yin deficiency and internal internal heat syndrome is diabetic retinopathy and diabetic carotid artery disease.The most common complication of Qi and Yin deficiency syndrome is diabetic carotid artery disease.The most common complications of Qi and Yin deficiency and blood stasis syndrome are diabetic lower extremity vascular disease,and diabetic carotid artery disease.The most common complication in patients with wet and heat trapped spleen and blood stasis syndrome and Yin and Yang deficiency syndrome with blood stasis syndrome is diabetic lower extremity vascular disease.7 The distribution of TCM syndrome types of various complications: there was no statistically significant difference in the distribution of TCM syndromes of each complication(P>0.05).Conclusion:1 Patients with type 2 diabetes mellitus combined with insomnia are more common in women,middle-aged patients,and patients with a disease duration of more than 10 years.Urban residents are more common.2 In this study,Qi and Yin deficiency and blood stasis syndrome were the most.According to the age from large to small,the duration of disease from long to short is Yin and Yang deficiency syndrome with blood stasis syndrome,Qi and Yin deficiency and blood stasis syndrome,wet and heat trapped spleen and blood stasis syndrome,Qi and Yin deficiency syndrome,Yin deficiency and internal internal heat syndrome.3 The age of the disease is relatively large,and the long-term syndrome is mostly Yin and Yang deficiency syndrome with blood stasis syndrome.The blood sugar is generally poorly controlled and the number of complications is the highest.The blood lipids in the wet and heat trapped spleen and blood stasis syndrome are generally poorly controlled.4 In this study,diabetic lower extremity vascular disease became the main complication.
