ObjectiveWe aimed to observing the rehabilitation influence of athletic therapy among patients with lumbar disc herniation(LDH)(qi stagnation and blood stasis type), and detecting the mechanism of athletic therapy in the treatment of LDH. Our purpose was to propose a simple, effective, economical and safety method of conservative treatment of patients with LDH.MethodsA total of100included cases with conservative treatment of LDH(qi stagnation and blood stasis type)were gathered for a prospective study in my tutor outpatient clinic (The first affiliated hospital of guangzhou university of Traditional Chinese Medicine orthopaedic clinic)from January2013to September2013. They were randomly divided into treatment group (the athletic therapy group) and control group(the traditional back muscle exercise group), every group had50cases.The two groups of patients were adminstered oral Yaobi Tong capsules(3granules/time,3times/d, took it after a meal,30days for a course of treatment) as the foundation treatment as the start of the study. The treatment group treated with the athletic therapy and Yaobi Tong capsules, while the control group treated with the traditional back muscle exercise and Yaobi Tong capsules for3course of treatment, and all took it half an hour after the meal. All patients were followed up for3months after treatment.We gathered the VAS Score, JOA Score, SF-36Scale, Oswestry Disability Index (OID) and Composite Score Index of pre-intervention and post-intervention in both groups as well as adverse reactions were recorded. We used chi-square test for enumeration data, t test for measurement data (represent as mean±standard deviation), and nonparametric Wilcoxon test for graded data with SPSS13.0statistical data processing system(95% confidence interval). When P<0.05, we considered as statistically significant.ResultsWe excluded out2patients who took other drugs (No.6, No.86) from our study and each one in the treatment group and control group. We collated the relevant data for statistical analysis3month later. At the end of the study, each group had1case (No.49, No.91) for Lost to follow-up, one case (No.74) in the control group who was performed surgery due to condition aggravation. The remaining95patients were collected for final statistical analysis without adverse reaction and serious complications.When the study was finished, we performed second assessment to all patients after treatment and data comparison was made of the two groups, the results were as follows:Firstly, in the VAS Score aspect, the treatment group of VAS Score(1.48±1.34) showed advantage in the terms of pain relief compared to the control group (3.23±1.76) and it revealed significant statistical differences difference(P<0.05).Secondly, in the JOA Score aspect, the treatment group and control group respectively obtained25.40±2.10scores and22.81±3.62scores. Score improved index respectively was6.02±2.50and4.41±1.31and improvement rate was89.6%and38.3%. The two groups had significant statistical differences difference in JOA score (P<0.05), which showed that the treatment group was better than the control group in terms of the efficacy.Thirdly, after the study, in the SF-36Scale aspects, in score of PF, RP, BP, GH, VT, SF, RE, MH, PCS and MCS, the treatment group scores respectively were75.0±5.7,73.4±16.6,65.3±9.3,58.8±4.9,77.5±4.9,71.5±6.6,75.3±14.4,76.7±4.6,66.8±6.1and76.8±6.0, and the control group scores respectively were49.1±5.8,51.6±12.1,43.6±7.1,53.7±3.5,63.6±3.7,56.1±6.9,66.9±12.1,59.5±3.8,50.0±4.5and60.4±3.4. The two groups had significant statistical differences difference in the two sets of data(P<0.05), which showed that the treatment group was better than the control group in terms of improving the patients quality of life.Fourthly, in the ODI Index aspect, the treatment group and control group respectively was22.58±17.52and47.83±16.16, and there existed significant statistical differences difference(P<0.05), which showed that the treatment group obtained better outcomes than the control group in terms of functional recovery.Fifthly, at the end of the study, the two groups were given the Comprehensive Score Index evaluation. In the treatment group, there were32excellent results of48cases which accounted for66.67%,14cases had good results which accounted for29.17%,2cases had fair results which accounted for4.17%, and no poor result case, and the improvement rate was95.8%. In the control group, there were9excellent results of47cases which accounted for19.15%,29had good results which accounted for61.7%,7had fair results which accounted for14.89%,2had poor results which accounted for4.26%, and the improvement rate was74.5%. The two groups had much more significant differences in the rate of excellent and good results (P<0.05), which showed that the treatment group was better than the control group in terms of the curative efficacy.Sixty, at the end of the study, the treatment group respectively was assessed with the Comprehensive Score of age, gender and degree of pain. The excellent rate of adults and middle-aged patients respectively was96.7%%94.4%. The excellent rate of male and female patients respectively was93.1%and100%, and there were no statistically significant difference between them (P>0.05), which showed that the age and gender of the patients had no significant effect on exercising effect. But the mild, moderate and severe pain in patients with good rate respectively was100%,100%and81.8%%, there was much more statistically significant difference between them (P<0.05), which showed that the degree of pain of the patients had much more significant effect on exercising effect, so the lighter the pain of patients with more significant effect.ConclusionOur study concluded like follows:Firstly, The athletic therapy had certain therapeutic effect of pain relief, functional recovery and improvement of quality of life on patients with LDH.Secondly, Gender and age of patients with LDH had no obvious impacts to the rehabilitation efficacy of the athletic therapy. Consequently, we recommended the athletic therapy application for LDH patients for all ages and different gender. Thirdly, the athletic therapy method does play a role in promoting the rehabilitation of patients with LDH with the advantages of simple, effective, economic, safety and wide range of applications, so it worth further study and promotion.
