Objective To evaluate the risk factors of cognitive impairment(CI) in patients withtype2diabetes,and to provide the clinical basis of early intervention,slowing andreducing the incidence of dementia.Method Totally100patients with type2diabetic were selected and all the patientswere from the department of Endocrinology,first clinical hospital of Shanxi MedicalUniversity.All the patients were tested cognitive function by the Montreal CognitiveAssessment (MoCA),According to the MoCA score,the patients were divided intotwo groups:cognitive impairment group(53cases,<26scores) and non-cognitiveimpairment group(47cases,26scores).Collected the information of diseasehistory,personal history,family history,measurement of systolic and diastolic bloodpressure,body mass index(BMI), tested glycosylated hemoglobin (HbA1c), fastingblood glucose (FPG), three consecutive days7glucose (blood sugar beforemeals, after meals2hours blood glucose, blood glucose before bed),bloodlipids(cholesterol, triglyceride, low-density lipoprotein cholesterol, high-density lip-oprotein cholesterol) and urinary albumin/creatinine ratio (UAIB/Cr).Results There were statistically significant differences in age,diabetic course,2hpostprandial glycemia,blood glucose fluctuation,triglyceride and UAIB/Cr betweenthe CI group and NCI group(the test statistics: t=3.264,Z=-3.102, Z=-2.093, t=6.199,Z=-2.096, Z=-2.466,P<0.05).Multiple regression analysis showed that the age andthe fluctuation of blood glucose lever had obvious correlation with MoCA scores,and the regression equation was Y (MoCA score)=36.363-0.664(glucosefluctuation)-0.115(age). Conclusion The age,diabetic course,2h postprandial glycemia,blood glucosefluctuation,triglyceride and UAIB/Cr might be risk factors for cognitive impairmentin type2diabetic patients.We can test the indexes to detection of risk factorsexist,control of blood sugar,reduce the blood glucose fluctuation level and controlthe blood lipid,so as to reduce the occurrence and development of cognitiveimpairment and improve the quality of life of patients and reduce the mental burednof patients and economic pressure.
