Objective:This study was completed to investigate the correlation between the TCM syndrome ofmigraine and the index of transcranial Doppler ultrasonography(TCD),to improve the accuracy ofsyndrome differentiation of the TCM of migraine.Methods:We selected the diagnostic criteria of the TCM syndrome of migraine,reference to the bookof TCM clinical research guidelines,issued in2002.The TCM syndrome of migraine was dividedinto five types,including hyperactivity of liver-YANG, phlegm headache, nephrasthenia headache,headache with blood stasis and deficiency of Qi and blood. We selected the western diagnosticcriteria of migraine,reference to ICHD-Ⅱ R1.We selected150patients of migraine attack,according to the inclusion criteria and exclusion criteria, and observed maximum velocity(Vmax),mean velocity(Vmean), pulsatility index(PI), resistance index(RI) and spectral shape of middlecerebral artery(MCA), anterior cerebral artery(ACA), posterior cerebral artery(PCA), vertebralartery(VA) and basilar artery(BA) of the side of pain of head by TCD. Additionally,we randomlyselected20normal persons, observing the above index of TCD. After the end of theobservation,we processed data by statistical analysis software, SPSSl8.0.Results:There was statistical difference(P<0.05), compared the PI(1.72±0.44) of MCA of the groupof headache with blood stasis with the PI of MCA of other groups.There was statisticaldifference(P<0.05), compared the Vmax(128.89±5.84cm/s) of BA of the group of headache withblood stasis with the Vmax of BA of other groups.There was statistical difference(P<0.05), compared the Vmean of ACA,MCA,PCA,VAand BA(successively:41.83±6.67cm/s,52.30±8.36cm/s,40.57±3.92cm/s,39.83±4.57cm/s,39.35±7.43cm/s) of the group of deficiencyof Qi and blood with the Vmean of ACA,MCA,PCA,VA and BA of other groups.There wasstatistical difference(P<0.05), compared the Vmax(162.43±17.50cm/s) of MCA of the group ofhyperactivity of liver-YANG with the Vmax of MCA of other groups.There was statisticaldifference(P<0.05), compared the Vmax(138.10±17.14cm/s) of ACA of the group ofhyperactivity of liver-YANG with the Vmax of ACA of other groups.There was statisticaldifference(P<0.05), compared the RI of MCA and PCA(successively:0.87±0.12,0.93±0.09) ofthe group of phlegm headache with the RI of MCA and PCA of other groups.There was statisticaldifference(P<0.05), compared the Vmax(118.07±29.58cm/s) of PCA of the group ofnephrasthenia headache with the Vmax of PCA of other groups.Conclusion:The troppo increase on index of TCD of Vmax and Vmean represented cerebral vasospasm.Headache with blood stasis was mainly with the troppo increase on Vmax of BA and PI of MCA.Hyperactivity of liver-YANG was mainly with the troppo increase on Vmax of ACA and MCA.Hyperactivity of liver-YANG was mainly with the troppo increase on Vmax of ACA and MCA.Nephrasthenia headache was mainly with the troppo increase on Vmax of PCA.The aboverevealed the correlation between the the above syndromes of migraine and cerebral vasospasm.Deficiency of Qi and blood was mainly with the troppo decrease on Vmean ofACA,MCA,PCA,VAand BA. Phlegm headache was mainly with the troppo increase on RI ofMCA and PCA. There was no correlation between the two syndromes of migraine and cerebralvasospasm,although the two syndromes of migraine had features on index of TCD.Consideringother reasons. By using the index of TCD,we could improve the accuracy of syndromedifferentiation of the TCM of migraine.
