Migraine is a disease with the feature of intermittent headache. There were a couple of theories to expound the pathogenesis. It is a common and frequently encountered disease with recurrent attacking and hardly cured. Migraine ranked in the top20from the years lived with disability of the WHO report in2001. It is defined as a most cripple chronic disease. The morbidity is985.2/100thousand in our country from the epidemiological survey. The site of the disease was in qi and blood, main and collateral channels and brains under the theory of TCM. Today there were no fundamental solution. Preventing attacks and symptom controlling were the commonly used treatment. But migraine treated under the TCM theory has a greatly effective in treatment and less side effect, varied approaches.Part one Literature ResearchThis paper summarized the name, pathogenesis, treatment of migraine in TCM and modern traditional Chinese medical physician’s achievements. It alao summed up the domestic and foreign scholars’s progress on the epidemiology, etiology, pathology, pathogenesis, diagnosis and treatment, and the research trend and direction of migraine, in our country or abroad. Currently the factor and pathogenesis are not confirm. It is said that the pathogenesis was complexity. The lack of recognition of pathophysiology. We know that there was contributor in most chronic pain diseases. So there were no special method to cure it. Pathogenic headache related to wind, cold, hot, damp, dry and fire in TCM. Internal damage headache relate to liver, spleen and kidney. Part two clinical and experimental research1. Objective:In order to supply the theoretical and practical basis, we treated it under TCM syndrome and explore the pathogenesis though clinical observation.2. Methods:128patients who comply with a standard were collected. An d80healthy persons as the control group. The survey include general condition, relevant factors(the imbalance of seven human emotions, heavy diet, drinking, smoking and troubles excessive), the scoring of migraine, the character of headache, clinical manifestation and TCM syndrome.3. Results:1)There were128patients colleted. The ratio of male and female is1:1.41. The average age was41.33±18.92years old. Mental workers had a higher risk than manual worker in the survey. Also housewife had the higher risk than manual worker. There were some influence in change of weather, especially in rainy day.2)Seven human emotions can affect the pathogenesis in a certain extent. The heavy diet would affect the migraine too and is the risk factor in positive correlelation. The more drinking the most chance to be afforded the migraine. Overtied could also influence the incidence of disease. All above elements had the statistical difference. But smoking would probably not affect the disease, with no statistical difference.3)The average score of headache was7.41±2.49, showing that in moderately severe degree. There was no difference between man and women. The evaluation of migraine disability degree showed that female had a higher score than male. And it showed that the elder the higher scoring. Temporal pain occupied49.2%of128patients and then was occiput, thirdly was backside. The headache properties was paroxysmal pain, heavy pain, dull pain and distending pain. It was rare to see empty pain and gall.4) In the study, the top5symptoms were dizziness (49.2), insomnia (32.0), dysphoria (30.5), dired (28.1) and dysphoria in chestpalms-soles (25.0) phlegm-turbidity disturbing mind was most commonly syndrome in128patients occupying32%. Liver-yang hyperactivity syndrome contained25.8%. Qi-blood deficiency was uncommon syndrome in migraine.4. Conclusion:The pathogenesis was complexity in migraine. The imbalance of seven human emotions, heavy diet, drinking, troubles excessive and weather variations were the induced factors. Smoking had a little effect on the disease. Phlegm-turbidity disturbing mind and liver heat depress in were the main two TCM pathogenesis. Invigorating spleen for eliminating dampness, clearing heat and eliminating phlegm and comfortable live qi were three main treatments in curing the migraine.
