ObjectiveTo explore clinical effects of dysphagia after stroke treated by "Tiao Shen Li Yan" acupuncture therapy through a randomiaed controlled trial, and also to verify the superiority of "Tiao Shen Li Yan" in treating dysphagia after stroke.MethodsSeventy patients who suffered from dysphagia after stroke have been s elected according to eligible criteria. All these patients are from Acupu ncture Department and Nerve Medical Department in Guangdong Province Trad itional Chinese Medical Hospital. They are randomly divided into two equa1working groups:one is the study group, the group of "Tiao Shen Li Ya n", the patients in this group will be acupunctured DU-20, DU-24, EX-HN-3, Shang-Lian-Quan, GB-12, GB-20, BL-10, LU-7, KI-6points mainly. The ot her group is the control group, which mainly acupunctures GB-12, GB-20, S J-17, PC-6, HT-5, DU-26, RN-23points. Both of them are assisted with EX-HN-11points and the posterior pharyngeal wall by pricking method. Two gr oups are conducted on the basic of routine medicine and recovery therapy. The treatment schedule is20minutes each time,6visits per week, to tally2weeks for about9-12times. Standardized Swallowing Assessment (S SA) will be used as a key indicator to assess the therapeutic effect of s wallowing function. What’s more, Water Swallow Test, Mini Nutrition Asses sment (MNA) and Videofluoroscopic Swallow Study (VFSS) will be used as a secondary indicator, the Videofluoroscopic Swallow Study (VFSS) will be u sed if patients have the wish to do it. All of these assessments will be evaluated before and after the treatment, and will be conducted by the sa me evaluator who don’t know patients’s group. Results1. In this RCT study, the gender, age, TCM syndromes, diagnosis, previous stroke and treatment, swallowing function and nutrition level are equal between two groups.2. Comparison of improving swallowing function between two groups:After the treatment, there are significant differences between the tw o groups (P<0.05). The score of SSA (21.700±3.097) is lower than the con trol group (23.450±3.247), P=0.009<0.05. It shows that "Tiao Shen Li Ya n" acupuncture therapy is superior to Routine acupuncture therapy in impr oving the swallowing function. After the treatment, the level of secondary indicator-Water Swallow Test in the study group are lower than the control group, but P=0.043, v ery close to0.05, it might due to the small sample size. Therefore, this outcome is not stable enough, it needs further study.3. Comparison of the Situation of nutrition between groups:After the treatment, there are differences between the two groups (P<0.05). The scores of MNA of the study group (23.273±3.357) are higher th an control group (21.069±3.560), P=0.015<0.05. The Comparison of levels of nutrition which assessed by MNA shows that the study group is better t han control group after the treatment (P=0.026<0.05). It means that the "Tiao Shen Li Yan" acupuncture therapy is superior to Routine acupuncture therapy in helping improving the nutrition level of the patients who suf fered from dysphagia after stroke. ConclusionsThe "Tiao Shen Li Yan" acupuncture therapy works more effective than Routine acupuncture therapy in improving the swallowing function and nut rition level in patients who suffered from dysphagia after stroke.
