ObjectivesWe launched a’H-NMR-based metabonomics study in case to find out the Mechanism of Xuefu Zhuyu decoction treatment on severe traumatic brain injury rats models. And evaluated the efficacy of Xuefu Zhuyu decoction through a30cases clinical study.MethodsThe96SD rats were randomly divided into3groups in’H-NMR-based metabonomics study:sham group, model group, Chinese medicine group (All rats from both model group and Chinese medicine group were impacted by the free fall object, sham group did not). The rats plasmas were detected with hydrogen nuclear magnetic resonance spectroscopy (’H-NMR) pop technique. And the detection data was statistically analysed.The30patients were randomly divided into2groups in clinical study: treatment group15cases(routine western medicine treatment,-a decoction of Chinese medicine/day), control group15cases(routine western medicine treatment). CK、CK-MB、CK-MB/CK、LDH、GLIU、ACHK、PRL、TSH in blood samples and Glasgow coma score、Glasgow outcome score were contrasted between the2groups.ResultContents of N-acetyl-aspartate(NAA)、γ-Hydroxybutyric acid(GABA)、 Creatine(Cre) were dropped in rats from model group compared to sham group, when those of Lactic acid (Lac)%Glutamate(Glu)、Choline(Cho) Rised。Contents of NAA、GABA、Cre were higher in model group than those in Chinese medicine group, the contents of Lac、Glu、Cho were lower in Comparison。Peak values of CK、CK-MB、CK-MB/CK、LDH、GLU、ACTH、PRL、TSH were reduced after Xuefu Zhuyu decoction treatment in clinical study, levels of which can be dropped earlier and recovered in14days. Glasgow coma scores and Glasgow outcome score were increased in treatment group.ConclusionMechanism of severe brain injury probably contains hypoxia ischemia brain damage、disorders of energy metabolism、cytotoxicity and neural excitotoxicity、cytomembrane and neuronal apoptosis after brain injury. Xuefu zhuyu decoction can ameliorate the ischemia and anoxic, reduce secondary brain insults, promote neural regeneration, increase Glasgow coma scores, improve the prognosis of brain injury.
