The Clinical Study Of Sanjin Granules In The Treatment Of The Postoperative Residual Stone After Percutaneous Nephrolithotomy

Objective:To explore the clinical curative effect of Sanjin granules in the treatment of postoperative residual stone after percutaneous nephrolithotomy(PCNL).Method:During the period of2012June to2013December, the patients who accept the operation of PCNL by the same operator from the guangdong NO.2TCM hospital uropoiesis surgical department. After the operation, all the patients to be anti-infection, hemostatic, nutritional support and other symptomatic treatment. Indwelling catheter1-3days, and review KUB or urinary tract ultrasound. Removal of the kidney fistula in7about days. The double J stent was removed at4weeks after the operation. According to the diagnostic criteria, inclusion criteria, exclusion criteria and exit criteria to select subjects.who have residual stones after postoperative review. The subjects were randomly divided into treatment group (group A) and control group (group B) by simple randomization method.Finally completed the study with22cases of group A,21cases of group B. The subjects of group A with Sanjin granules of oral after postoperative review. The drug composition:desmodium30g, lygodium japonicum20g, endothelium corneum gigeriae galli20g, seed of cowherb20g, pyrrosia leaf10g, achyranthes bidentata10g, Herba Dianthi10g, Polygonum aviculare10g, semen plantaginis10g, Molherwort10g, liquorice5g. Daily1agent. Using dose of Chinese traditional medicine formula particles mix thoroughly by warm boiled water, and take it twice for8weeks. At the same time, ask the patients drink water more than2000ml everyday, and in conjunction with jumping movement after removal of double J tube. The subjects of group A only be asked to drink water more than2000ml everyday and in conjunction with jumping movement after removal of double J tube. After treatment of4weeks and8weeks, the two groups respectively review KUB and ultrasound, and record residual stone’s size, location, quantity, discharge time, and the clinical symptoms. Evaluate their curative effect. Depending on the data, using SPSS17.0statistical software to analyze data by different statistical methods. Using the methods of t test, chi-square test, Fisher’s exact probability, Wilcoxon rank sum test to evaluate differences between groups.Results:①After4weeks of treatment, the cure rate and total effective rate of group A were4.55%and4.55%respectively,the cure rate and total effective rate of group B were4.76%and4.76%respectively. No statistically significant differences between the two groups (P>0.05).②After8weeks of treatment, the cure rate and total effective rate of group A were36.36%and63.64%respectively, the cure rate and total effective rate of group B were4.76%and4.76%respectively. The two groups had significant difference(P<0.05).③Comparison of curative effect in group A after the treatment of4weeks and8weeks respectively. The difference is statistically significant(P<0.05). The curative effect of treatment of8weeks is superior to the treatment of4weeks.④〤omparison of curative effect in group B after the treatment of4weeks and8weeks respectively. There was no statistically significant difference (P>0.05).⑤4weeks to discharge the stone of A, B two groups were7cases and3cases respectively.4-8weeks to discharge the stone of A, B two groups were7cases and1case respectively.8weeks still not to discharge the stone of A, B two groups were8cases and17cases respectively. Compare two groups of lithagogue time, with a significant difference (P<0.05).⑥The two groups had no occurrence of diarrhoea, palpitations,rash,low blood pressure, dizziness and other adverse symptoms.Conclusion:The effect of Sanjin granules on the residual stone has no obvious advantage within4weeks after PCNL. But4～8weeks after the surgery (after removal of the double J tube),it have the obvious advantages to promote the discharge of residual stone, In general, Sanjin granules can to discharge the residual stone more effectively and quickly after PCNL in a certain period of time. And it is security, has no obvious adverse reactions, and can be widely used in clinical practice.
