Objective:To study the neoadjuvant chemotherapy in the treatment of rectal cancer, to explorethe expression changes and clinical significance of the tumor stem cell marker CD44Methods:49patients with rectal cancer from the general surgery department of GeneralHospital of Beijing Military Region from2012.05to2013.08were analyzedretrospectively.49patients were randomly divided into research group (22cases) andcontrol group(27cases) after digital rectal examination, endoscopic ultrasonography, CTscan, MRI and PET-CT,endoscopic and pathological biopsy diagnosed with colorectalcancer.All the patients treatment were not treated with chemotherapy and radiotherapybefore, and tumor staging and treatment plan are determined.22cases of study groupwere treated by neoadjuvant chemoradiotherapy followed by mesorectal excision (TME)after6-8weeks.Cancer primary tissue, tissue with in10cm from the cancer tissue andlymph node tissue of every patient with immunohistochemical SP method to detect thethe expression of CD44in primary colorectal cancer tissues and normal tissues of studyand control group. Analysis clinical significance by explore lymph node metastases andtumor invasion depth.Results:1.CD44expression had no significant difference in colorectal carcinoma lesionsand normal margin tissue of cancer of study group (P>0.05). But in the control group,CD44expression was significantly different (P <0.01).2. Compared with the control group, expression of CD44in study group wassignificantly different(P <0.05).3.the expression of CD44positive cases appear to have a statistically significantdifference in the number of lymph node metastasis cases (P <0.05). 4.tumor invasion depth was statistically significantly different in CD44positivecases both in study and control group (P <0.05).Conclusion:1.As a specific marker of cancer stem cells, CD44plays an important role inpromoting tumorigenesis, invasion and metastasis. Detection of CD44v6expression canbe used as an important index for diagnosis, metastasis and prognosis of colorectalcancer.2.New adjuvant chemotherapy can reduce the expression level of CD44in rectalcancer tissues, reduce the invasion and metastasis of rectal cancer cells; At the sametime, New adjuvant chemotherapy improves sphincter preservation rate and long-termlife quality of patients.3.Cancer stem cells can promote tumor recurrence and metastasis, furtherdevelopment of targeted therapies for targeting cancer stem cells and tumor cellmicroenvironment, strive for maximum control and kill cancer stem.
