Application And Value Of Neoadjuvant Chemotherapy In Treatment Of Middle And Low Stage Advanced Rectal Cancer

Purpose:To explore the application of neoadjuvant chemotherapy in the treatment of low rectal cancer,and to guide the clinical treatment.Methods:Retrospective analysis September 2015-September 2017 Department of Gastroint-estinal and Rectal Surgery,82 patients with low rectal cancer patients,that is,the lower edge of tumor lesions were 3cm-6cm from the anus.According to different treatment methods,were randomly divided into treatment group and control group.The treatment group was neoadjuvant chemotherapy + operation group,31 cases,the control group was direct operation group,51 cases.The basic clinical data,operation time,intraoperative blood loss,anal sphincter preservation,improvement of tumor staging,postoperative complications,anal function recovery,short-term recurrence and quality of life were compared between the two groups.Results:Both the experimental group and the control group underwent laparoscopic radical resection of rectal cancer with R0 resection,and both groups had no perioperative death.Neoadjuvant chemoradiotherapy can obviously kill tumor,which can soften and shrink the tumor,and achieve the tumor reduction stage.The TNM staging in the experimental group before and after neoadjuvant therapy has statistical difference(P=0.00 < 0.05).The new adjuvant therapy could extend the distance of the lower margin of the tumor and improve the retention rate,and the anal rate of the experimental group and the control group was 78.7% and 58.7%,both of which had statistical significance(P=0.037 < 0.05).In the three months after the operation,the anal function of the two groups in the two groups was well received,which could achieve the autonomic control and defecation,and the difference was not statistically significant(P > 0.05).There was no statistically significant difference between the two groups in operation time,intraoperative hemorrhage and postoperative complications(P BBB 0 0.05).In the six months after the surgery,2 cases of local recurrence of tumor in the treatment group and 5 cases of local recurrence in the control group were observed.The recurrence rate of the experimental group was significantly higher than that in the control group,and the local recurrence rate was 3.2% and 9.8% respectively.Conclusion:Neoadjuvant therapy helps to improve the low phase progress in the clinical treatment effect of patients with rectal cancer,tumors can be reduced,can significantly improve the patient’s anus rate,reduce local short-term recurrence rate,improve patient’s quality of life,did not increase the operation time,intraoperative blood loss and the risk of postoperative complications.Neoadjuvant therapy is worth popularizing and applying in the comprehensive treatment of low-and middle-stage rectal cancer...
