Objective To study the correlativity of the serum levels and genepolymorphism in some kinds of cytokine including tumor necrosis factor-a (TNF-a),interferon-γ(IFN-γ) and interleukin-4(IL-4) and tuberculosis by analysis on thechanges and regularity in different phase of tuberculosis and healthy controls toclarify the relation in TNF-a, IFN-γ and IL-4with pathogenesis,development andprognosis of tuberculosis; to investigate the correlativity of gene polymorphisms ofIL-4T590C and TNF-a G308A with the susceptibility of tuberculosis in QinghaiTibetan population. Methods25cases of patients with active phasetuberculosis,25cases of patients with non-active phase tuberculosis,25cases oftuberculosis patients who received3months treatment of anti-tuberculosis and25cases of healthy controls were selected to detect the serum levels of TNF-a, IFN-γand IL-4by means of enzyme linked immunoabsorbent assay (ELISA); and25Tibetan patients with tuberculosis and25healthy controls were selected to investigatethe susceptibility of tuberculosis by means of polymerase chain reaction-restrictionfragment length polymorphism (PCR-RFLP) method. The statistics were carried outby variance analysis and two-two comparison of SNK analysis. The survey subjectivewere come from out-patient department and in-patient diagnosed cases of infectiousdiseases hospital Qinghai province and the25healthy controls were come from thesame hospital who receiving normal examination. Results The TNF-α levels ofpatients with tuberculosis in active stage or in non–active stage or anti-tuberculosistreatment for3months and the healthy controls were0.156±0.070ng/ml,0.082±0.031ng/ml,0.106±0.019ng/ml and0.058±0.012ng/ml,respectively, andshown significant difference between the two groups (P<0.05). The serum levelsof IL-4in groups mentioned above were0.150±0.047ng/ml,0.074±0.023ng/ml,0.028±0.013ng/ml, and0.025±0.008ng/ml, respectively. There was nosignificant difference shown between anti-tuberculosis treatment group and healthycontrol group, but there was a significant difference between tuberculosis in activestage and non–active stage (P<0.05). the genotypes of IL-4T590C TT, CT and CC incontrol group was56%,36%and8%,respectively; whereas in tuberculosis group was64%,32%and4%, respectively. Compared with IL-4T590C TT and CT, the CCgenotypes was shown a lower expression in all of the groups, but there was noobvious difference between control group and tuberculosis group(P>0.05). The IL-4T590C gene frequency of C and T in control group was0.74and0.26, respectively, intuberculosis group was0.80and0.20, respectively. there was no significant differencebetween the two group(sP>0.05). the genotypes of TNF-α G308A GG, GA and AA incontrol group was64%,28%and8%,respectively; whereas in tuberculosis group was 60%,28%and12%,respectively. Compared with TNF-α G308A GG and GA, the AAgenotypes were shown a lower expression in all of the groups, but there was noobvious difference between control group and tuberculosis group(P>0.05). TheTNF-α G308A gene frequency of G and A in control group was0.780and0.222,respectively, in tuberculosis group was0.74and0.26, respectively. there was nosignificant difference between the two groups(P>0.05).In summery, there was nodifference between tuberculosis and control group involved gene polymorphism inthe present study. Conclusion The concentration of serum IL-4and TNF-α inpatients with tuberculosis is closely related with its development, clinical outcomeand prognosis. Therefore it shows some value in detecting serum IL-4and TNF-α forevaluation on diagnosis and prognosis of tuberculosis. There is no obvioussignificance in the gene polymorphisms of IL-4T590C and TNF-αG308A betweentwo groups, so it estimated that the two kinds of gene polymorphisms are not relatedto the susceptibility of tuberculosis.
