ContextThe worldwide input in mental health is seriously insufficient.Compared withother clinical disciplines need hardware supporting, mental health services needmore human resources related investment. Community mental health service, as partof the mental health services, is gradually development, needs more attention andinvestment proportion. Guangzhou current mental health resources are nowdeveloping and reasonable abundant.Little is known about the basicinformation(geographic or treatment)and human resource of Guangzhou mentalhealth service system,especially in community based mental health care.ObjectiveTo estimate the community mental health service resource configuration andcommunity mental health service staff demand in this Guangzhou MentalCommunity Mental Health Resource Baseline Survey Initiative.For providingeffective resource data to formulate policies, and supporting scientific referencebasis for subsequent research.MeasuresThe survey is mainly aimed at the community mental health resources inGuangzhou. Community mental health sector management part adopts theGuangzhou public health for mental health service package repository for writtenexamination indicators related to collection. Mental health workers ability demands part adopts questionnaire of the general situation of demographic data collection,using the Minnesota satisfaction questionnaire (MSQ-20), the world healthorganization five indicators of physical and mental health scale (WHO-5) to evaluateworker working attitude.198community mental health institutes, and238community doctors, conducted from September1st to November31st,2013inGuangzhou,China.ResultsInvolved in the investigation of community mental health departments ofeffective questionnaires were198(96.6%). Community mental health institutionsmanagement of37.55km2in average area,and service for the average population of64,200people in Guangzhou. Guangzhou heavy mental disorders detection rate was3.51‰, which district adjust the standard (more than4‰) seems to be CongHua(5.75‰), Yuexiu district (5.45‰), Liwan district (5.23‰), Huadu district (4.10‰).Detection rate may be affected by the immigration population.Participate in the survey of238community essence of the doctor, men of155women (65.1%),155(34.9%). Sex ratio1.87:1; Mean age:(35.18±0.57)year-old, p<0.01. There are191(80.3%) registered permanent residence, the Guangzhouaccount for47(19.7%), people with psychiatric right to prescribe account for39(16.4%),24(10.1%) showed a work shift intention in Guangzhou.Common problems of community mental health departments are: communitymental health work insufficient funds (78.2%), mental health related professionalguidance is insufficient (60.5%) and large intensity of work (56.7%), communitymental health workers, low economic status (54.2%).Community doctors score the WHO-5(10.56±5.57), which score13or higherthan are accounted for38.7%of the total; All items MSQ-20score: internalsatisfaction subscales (40.43±4.87) points, external satisfaction subscales(19.15±2.39) points, satisfaction with working conditions (20.60±2.53). Univariateanalysis was used to external satisfaction scores and the WHO-5happiness scalescores were positively correlated (r=0.98, p<0.01). Intrinsic satisfaction scores andnegatively correlated with intention of participating investigators transfers (r=-0.69, p<0.01). The female community doctor will score higher in intrinsic thanmale(t=-1.912,P=0.018).The factors will have effect in the intention of participatinginvestigators transfers might be the satisfaction with working conditions scores[OR=0.62,95%CI(0.24~2.28)], the intrinsic scores [OR=2.11,95%CI(1.06~2.85)],and the responsibility scores[OR=1.18,95%CI(0.19~1.58)].ConclusionsStructural barriers exist to this reallocation.Reallocation of treatment and humanresources could decrease the problem of unmet need for treatment of mentaldisorders among serious cases. Careful consideration needs to be given to the valueof community mental health resource, especially human resource. Severe mentaldisorders detection rate, which may be associated with many factors, such asimmigration population; Community mental health workers overall working stationis still needs to be improves, the low level of compensation and tend to work shiftshould be notice.Need to increase the professional and technical supporting for thecommunity,and to improve working conditions, and cooperate with the supervisionof the psychological, so that we might improve the level of community service andthe worker’s enthusiasm.
