BackgroundKawasaki disease is a kind of disease with systemic vasculitis in pathological chan-ges. It can cause coronary artery dilation and coronary aneurysm.At present,Kawasakidisease is one of the most common acquired heart diseases in children.Early diagnosisof Kawasaki disease can improve the prognosis,but there is no sensitive diagnostic in-dices. As a new inflammatory marker, secretory phospholipase A2is considered as anindependent prediction factor in cardiovascular events.This study aimed to study thechanges of sPLA2in the early phage of Kawasaki disease,and to explore the signific-ance of sPLA2in early diagnosis of Kawasaki disease.MethodsThis experience was divised four groups,24patients with Kawasaki disease wereenrolled as KD group,18patients with respiratory infection as infected group,19pa-tients with Henoch-Schonlein purura as vasculitis group,and22children with healthycheck-up as healthy group.Four groups of patients after admission that blood sPLA2,C-reactive protein,erythrocyte sedimentation rate,procalcitonin and routine blood,kidney and liver function,myocardial enzymes, et al.At the same time,take the chestX-ray and electrocardiogram.Patients were enrolled as KD group all have echocard-iography detection.ResultThe level of sPLA2in the acute phase KD group were significantly higher than theother groups,(P<0.05).The level of sPLA2in the group of coronary lesions was sli-ghtly higher than the group of no coronary lesions,but the elevated level was no signi-ficant,(P>0.05). The level of sPLA2in the group of typical Kawasaki disease and th-e group of incompletely Kawasaki disease was no significant differences,(P>0.05).The level of sPLA2in the KD group was positively relative to the level of CRP andthe level of ESR,(P<0.05).But it was not relative to the level of PCT,(P>0.05).Co- mpare the level of sPLA2with CRP and ESR in ROC curve, At the cutoff value of44.59ng/ml, it shows better sensitivity and specificity than CRP and ESR in diagnosisof Kawasaki disease.ConclusionsPLA2can be used in diagnosis of the early stage of Kawasaki disease.It has bettersensitivity and negative predictive value than CRP and ESR in early diagnosis of Ka-wasaki disease.
