Research On Evaluation Of The Effectiveness Of Regulations Over The New Rural Cooperative Medical Fund

The new rural cooperative medical system has played an important role in guaranteeingfarmers’ access to basic health services and abating farmers’ illness-related poverty. Butirregular entry fees collection and funds arbitraging have appeared in the operational processof new rural cooperative medical system constantly, highlighting the necessity and urgency ofthe effective supervision of the new rural cooperative medical fund. Based on this background,this thesis performs analysis of the establishment of the new rural cooperative medical fundsupervision effectiveness evaluation index system, the method selection and its application.This will play an important role in improving the effectiveness of the new rural cooperativemedical fund supervision.This thesis is divided into seven chapters. The first chapter is the introduction. Thesecond chapter is the basic research, mainly including the related concept definition andtheoretical basis. The chapters three to six are the main part of this thesis, involving theestablishment of new rural cooperative medical fund supervision effectiveness evaluationindex system, the method selection, the empirical analysis and the corresponding suggestions.Chapter seven is the conclusion of the research and put forward the questions remaining to beresearched.The main conclusions of this thesis are generalized as follows:（1）Based on clearing the basic implications and theoretical basis of the new ruralcooperative medical fund supervision effectiveness, this thesis clarifies the functions of thenew rural cooperative medical fund supervision effectiveness evaluation, including thefunction of description, analysis, evaluation, supervision and guiding; the thesis also designsthe steps of the new rural cooperative medical fund supervision effectiveness evaluation.（2）Based on clearing the target and basic principles of the new rural cooperativemedical fund supervision effectiveness evaluation index system, this thesis uses the Delphimethod to screen indexes, and the analytic hierarchy process (AHP) with experts reliability todetermine the index weight, and forms a complete new rural cooperative medical fundsupervision effectiveness evaluation index system eventually, including3first-level indexes,6second-level indexes and26third-level index. It is believed that indexes covered contents are comprehensive, the method to establish index system is reasonable and index weights arecalculated accurately through analysis.（3）After clarifying the principles of selection of the new rural cooperative medicalfund supervision effectiveness evaluation index system, this thesis compares the advantagesand disadvantages and applicable situations of several typical modern comprehensiveevaluation methods such as data envelopment analysis method, artificial neural network, graycomprehensive evaluation method and fuzzy comprehensive evaluation method, states thatthe evaluation process of the new rural cooperative medical fund supervision effectivenesswill involve a lot of complex situations and many factors which are difficult to quantify, andthere are no absolutely clear boundaries of their evaluation grading. This fuzziness makes theother three kinds of evaluation methods unreasonable, and fuzzy comprehensive evaluationmethod can quantify the factors which are difficult to quantify using the membership degreetheory of fuzzy mathematics.This method can solve this problem systematically andcomprehensively, therefore, fuzzy comprehensive evaluation method is selected as anappropriate method. And then the thesis states the calculating process of fuzzy comprehensiveevaluation method according to the design of new type of the new rural cooperative medicalfund supervision effectiveness evaluation index system.（4）This thesis evaluates the situation of Qingzhou City of Weifang, Wenshang Countyof Jining and Shizhong District of Zaozhuang using the new rural cooperative medical fundsupervision effectiveness evaluation index system and fuzzy comprehensive evaluationmethod, then calculates the new rural cooperative medical fund supervision effectivenesssynthetical evaluation values of the three counties. The values are17.544,16.024,17.504,illustrating the fact that among the new rural cooperative medical fund supervisioneffectiveness of the three examples, Qingzhou City of Weifang is the strongest, ShizhongDistrict of Zaozhuang is in the middle and Wenshang County of Jining is the weakest. Thethesis also compares the supervision ability, the supervision efficiency and satisfactioncomprehensive evaluation score to find out the key factors effecting the new rural cooperativemedical fund supervision effectiveness strength, including the management rate ofinformatization, the equipment rate of law enforcement and forensics facility and theperformance of supervision work, etc. （5）For the problems exposing in the design of the new rural cooperative medical fundsupervision effectiveness evaluation index system and examples analysis, this thesis putsforward practical suggestions considering the actual situation of the new rural cooperativemedical fund supervision. Suggestions improving the effectiveness of the new ruralcooperative medical fund supervision mainly include: perfect the new rural cooperativemedical fund supervision system, strengthen the team construction and the informatizationconstruction of rural cooperative medical fund supervision to improve the supervision abilityof supervision department; strengthen risk awareness, overcome risk-taking behaviors, carryout on-site inspections actively and increase the penalties for the designated medicalinstitution to improve the supervision efficiency of supervision department; establish thesystem of information disclosure and the multi-level information publishing channels, set up acomplaints reporting system and a supervision and incentive mechanism to enhance thesatisfaction of supervision. In addition, some measures should also be taken to as assistancesincluding establishing and improving the construction of the agencies at the grassroots level,implementing county-level vertical management of township-level working staff of the newrural cooperative medical system and carrying out " honest hospitals in new rural cooperativemedical system" establishing activities, etc.
