Infectious diseases in China has been seriously endanger people’s health, infectious diseases. In order to grasp the dynamics of the epidemic of infectious diseases, to understand the characteristics and change trend of population distribution of infectious diseases, epidemic effectively control the outbreak of infectious diseases, protect the health of people, the Tai’an Municipal Health Bureau formulated the infectious disease surveillance scheme. For the effective implementation of convenient infectious disease surveillance program, Tai’an City Health Bureau disease prevention and control center proposed the establishment of the Tai’an Municipal Health Bureau immunization information management system, monitoring and information management of infectious diseases.In this paper, using UML modeling language, use case diagram, class diagram, sequence diagram, activity diagram, describes the design and implementation of the process planning information management system of Tai’an City Health Bureau immunity. Firstly, the system development background, and then the requirements elicitation, and using case analysis method and the use case diagram of the system functional requirements, the system function is divided into six modules:system setting, basic information, vaccination, immunization, disease surveillance and immune monitoring survey. The design of system architecture and program structure, and the class diagram and sequence diagram, and describes the system functions in detail, gives the system database table structure, system function interface, and using activity diagram describes the implementation process of part function, gives the system of function implementation code, finally has carried on the system overall test, the overall implementation of the system of monitoring information management of epidemic cerebrospinal meningitis.The system uses the B/S structure pattern, the design and development of integrated framework of J2EE multi-layer architecture based on Struts+Spring+Hibernate, not only to achieve the complete separation of view, controller and model, but also realizes the separation of business logic layer and persistence layer. Using SQLServer2008database management system for data storage, the immunization, vaccination investigation, disease surveillance and immune monitoring function module, realizes the infectious disease case survey information, vaccination and immunization level, close contacts of patients and other information to add, query, browsing, provides an electronic platform Tai’an City infectious disease surveillance information management.
