With the rapid development of society, people with high blood pressure, or other chronic diseases are increasing year by year. A growing number of medical studies show that majority of chronic diseases adhere to medication and improvement of individual lifestyle. Health care is beneficial to improve the effect of treatment, but also prevent other chronic diseases. In order to help patient rehabilitation as soon as possible, we need a comprehensive long-term monitoring for chronic diseases and provide scientific guidance of health management equipment. On the basis of such requirements, this article designed a health Management SBPM (Smart Blood Pressure Management System) System, for hypertensive represented by chronic diseases. Making use of the existing mobile communication technology, the SBPM runs on smart phone or mobile terminal, which can perceive personal state of the health and behavior of the management for hypertensive in anytime and anywhere.SBPM system collects blood pressure of patients, health assessment automatically. With location awareness, obtain the altitude, weather, temperature, etc. Combined with medical rules for data fusion, SBPM recommend health care program and life guidance for patients in the scenario. For example, SBPM analysis the blood pressure and risk factor level, combined with the location or weather temperature, and recommend specific health guidance, such as migrating to low elevation area, keeping warm or cool to control blood pressure. Furthermore, through physical activity and diet analysis, SBPM will give specific behavior guidance, according to the doctor’s advice. With the management of health care daily, the hypertensive will set up the health concept, and keep blood pressure stable. But also other chronic diseases will be prevented and cured effectively.This paper analyzed the needs of the hypertensive user, the architecture of the system, designed the data model. Preliminary established blood pressure status, risk factors and health guidance mechanism of three layers of inference engine. This application can be installed in smart phone or smart portable terminal. This app comes along with user’s normal life, same as a supervisor. Secondly, the pedometer algorithm of activity supervisor is improved. And on this basis, the preliminary data validation is presented. Due to the openness of Android, this system runs on it.At present, the research and implementation of this article is relatively brief, the subsequent studies will be further refined, for instance, the multi-user’s data analysis to improve the represented by hypertension, chronic health management services.
