Urban Elderly Community Visits Factors

Objective:Comprehending community clinic selection and utilization of the elderly through making a Shinan district survey about the elderly community clinic in Qingdao, finding the main influence factors on the elderly’s choice in the community clinic and exploring the differences of different demographic characteristics of the elderly about choose the community clinic in order to provide corresponding management strategies for the government, to optimize the allocation of health resources and alleviate the current situation of the difficulty in medical service and high cost of treatment, and to provide the better service decision-making reference for the elderly.Methods:Though formatting the survey questionnaire in the status and influence factors of the elderly community clinic and utilizing the method of multistage random sampling system for people over60years old from Qingdao districts10streets,55communities though an anonymous questionnaire survey and interview. We established database with excel then did statistical analysis with SPSS20.0software. Statistical analysis included descriptive statistics analysis, single factor analysis and multiple regression analysis, etc.Results:Utilization rate of the elderly community health service is not high in Shinan of Qingdao. The community clinic proportion is just42.22%under the condition of common disease of elderly. The elderly’s satisfaction of community health services is higher; the overall rate of satisfaction is81.59%. The satisfaction from high to low in turn is convenient community doctor degree (86.03%), community service attitude of medical personnel (62.54%), community medical expenses reimbursement procedures (58.41%), community medical circumstances (54.29%), community medical equipment (39.05%), medical technology level of community health workers (28.57%), community health service price (22.82%). The difference of elderly community clinic has no statistical significance in genders (P>0.05);The differences of age, culture level, living condition, economic income, employment, medical treatment pays, walking time, chronic illnesses, self-evaluation of health status in the community clinic choices have statistical significance (P<0.05); the dominance ratio of main influencing factors in the elderly community clinic from high to low in turn is health costs (OR=9.346), family income (OR=6.667), medical devices (OR=5.310), chronic disease prevalence (OR=4.405), the age (OR=4.386), mode of payment (OR=4.310), the elderly’s cultural level (OR=3.846), occupation (OR=3.401), self-evaluation of elderly health status (OR=3.165), environment for patients (OR=2.567), frequency (OR=2.457), the convenience degree (OR=1.198).Conclusions:The current utilization rate of community health service is not high. The level of community health service cannot satisfy the residents, particularly of the needs of the elderly residents. Relevant government departments should strengthen the construction of community basic health service system in recent years, especially for the support of elderly patients with community. The accessibility of community health service has obtained the large degree of increase. In addition, health costs, family income levels, choice of medical equipment are the elderly community clinic of main influence factors under the condition of the elderly with common disease.
