Syndrome Characteristics In Patients With Hyperuricemia 663 Cases With Metabolic Syndrome

Objective:To analysis TCM syndrome features of metabolic syndrome with hyperuricemia by multicentre and large sample study.And to provide further clinical support for the treatment of metabolic syndrome with hyperuricemia.Methods:We made the clinical TCM syndrome survey based on the interviews of experts.We used the standard of metabolic syndrome diagnostic criteria by IDF in2005;the TCM syndrome standard is based on People’s Republic of China TCM syndrome standards;the hyperuricemia diagnostic criteria is the Chinese Medical Association of Rheumatology criteria in2004.In this survey, we chosen5411cases from9centres during2010to2012. There are663cases of metabolic syndrome with hyperuricemia,4635cases of metabolic syndrome patients and776cases of normal people.We collect the patients information which including general information, anthropometric indicators, chemical examination and the four methods of diagnosis by the clinical cross-sectional survey of a large sample.The collected data will be double checked dual entry and be uploaded to the network database. The technical staffs export the data.We use the spss17.0statistical software to analysis the patients’anthropometric indicators and chemical examination.We use independent sample t test or ANOVA to analysis the measurement data and express it as the form of mean±standard deviation (x±s).The enumeration data is analysised by the chi-square test. We use the cluster analysis to summarize the high-frequency symptoms.The aim of this study is to conclude the type of the TCM syndromes of the metabolic syndrome with hyperuricemia in order to provide support for the further treatment of it.Results:1.General Information:There are663cases of metabolic syndrome with hyperuricemia.The metabolic syndrome with hyperuricemia patients showed an overall sex ratio of1.78male to1female. Males have a higher risk than females in this disease. In the age composition,65%of the metabolic syndrome with hyperuricemia patients are older than50years old, the mean age of them is53.83±11.52(years old).That means the main component of the metabolic syndrome with hyperuricemia patients are elder patients.2.Physical and chemical examination:Males have significant differences in triglycerides, high density lipoprotein, low density lipoprote in different groups.The TC(mmol/L) of the three groups are3.1±2.69,2.42±4.12,1.15±0.38, the HDL- C(mmol/L) of the three groups are1.16±0.4,1.2±0.37,1.34±0.28, the LDL-C(mmol/L) of the three groups are2.97±0.77,3.11±8.09,2.75±1.51.The TC and LDL-C of MS merge HUA group are higher than the normal group,and the HDL-C of it is lower than the normal group. What’s more.the TC of the MS merge HUA goup is even higher than the simple metabolic group. Females have significant differences in fasting blood glucose, triglycerides and low density lipoprote in different groups. The FPG(mmol/L) of the three groups are6.17±1.6,6.14±1.64,4.95±0.43, the TC(mmol/L) of the three groups are2.55±2.02,1.86±1.4,0.91±0.4, the LDL-C(mmol/L) of the three groups are3.13±0.86,3±0.79,2.56±0.61.3.The relationship between hyperuricemia and sex,age:Males and females of the group of MS and HUA have a significant difference in the level of hyperuricemia.Males hyperuricemia are473.64+116.892and females are413.71±70.822.However,the level of hyperuricemia has little difference between different age groups in statistics.4. The relationship between hyperuricemia and the component of MS:The percentage of the cases which have a high level of TG in the group of the metabolic syndrome with hyperuricemia is98.94%.The percentage of the cases which have a high level of FPG in the group of the metabolic syndrome with hyperuricemia is61.24%. The percentage of the cases which have a high level of Diastolic blood pressure in the group of the metabolic syndrome with hyperuricemia is67.12%. The percentage of the cases which have a high level of Systolic blood pressure in the group of the metabolic syndrome with hyperuricemia is67.12%. The percentage of the cases which have a low level of HDL-C in the group of the metabolic syndrome with hyperuricemia is44.19%. The level of hyperuricemia is different between the normal and abnormal group of HDL-C,the males hyperuricemia are481.06±143.548and461.71±48.286,females hyperuricemia are422.6±91.157,406.24±46.890.5.TCM Symptom:There are significant statistical differences among different groups in the symptom of stomach, liver, heart and brain, kidney and somatic. There are41symptoms which the frequency is higher than10%among the metabolic syndrome with hyperuricemia patients.The frequency is like this:The stomach symptoms are thirsty and like drinking is37.23%, lassitude is36.23%, heaviness in the body is34.76%, epigastric fullness24.88%, abdominal distention is23.20%, abdominal distention after eating is20.92%, Eat more and hungry is20.15%, sleepiness is1.87%, acid regurgitation17.10%, belching is13.58%, thisty but do not want to drink is12.92%;the liver symptoms:irritability is35.21%, sighing is15.72%, flank pain is15.23%, emotional depression is14.15%, chest threatening pain is12.50%, facial hongre is11.88%; the heart and brain symptoms:dizziness is41.74%, dreaminess is34.6%, forgetfulness is33.03%, insomnia is28.99%, palpitations is20.95%, headache is20.40%, chest tightness is17.00%, stretching is16.90%, feeling of fullness in the head is14.42%, heavy head such as wrap is11.85%; the kidney symptoms:low back pain is33.08%, backache is32.16%, tinnitus is21.47%, knees soft is15.36%, hearing loss is14.77%; the somatic symptoms:spontaneous is22.73%, numbness is20.61%, easy cramps is20.31%, joint pain is18.13%, body aches is16.00%, night sweats is15.69%, dysphoria in chestpalms-soles is14.29%, foot swollen is13.85%, hot flashes is10.00%.6.TCM signs:There are significant statistical differences among different groups in the signs of face,body,stool and urinate. The high-frequency symptoms of metabolic syndrome with hyperuricemia patients are huge belly(47.81%), abdominal tension(16.14%), abdominal sagging(9.95%), facial shine(15.8%),facial redness(8.14%); constipation(10.86%), loose stools(7.99%), poor stooi(7.54%) and urine redness(7.69%).7. Tongue and pulse:There are significant statistical differences among different groups in the existence of Tongue and pulse.Most of the metabolic syndrome with hyperuricemia patients’colour of the tongue is red(37.67%), purple(15.77%) or pale(15.16%).The shape of the tongue is big fat(33.18%,ecchymosis(10.11%) or varicose veins under the tongue(16.59%).The tongue is greasy(39.37%), thin(32.58%), thick(28.96%), dry or little fluid(10.26%).The pulse is slippery(39.5%), string(39.22%), veinlets(29.56%), deep(23.23%) or rapid(14.93%).Conclusion:We can conclude the TCM syndrome of the metabolic syndrome with hyperuricemia patients as five types.They are fullness with the phlegm,fullness within the virtual,Phlegm stasis. And it can also merge with the syndrome of:the deficiency of the kidney and liver yin and spleen crowed and liver stasis.
