Background:Leukorrhagia is a kind of common and frequently-occurring gynecological disease. The main clinical manifestations are the amount of vaginal discharge increase obviously, its color, quality, odor can be abnormal, associated with systemic or local symptoms at the same time. Leukorrhagia have the characteristics of chronic, protraction, easy to recur and so on. Without timely treatment, it will not only affect the menstruation and fertility, also seriously affect the women’s physical and mental health, so it can’t be ignored. Epidemiological survey found that the prevalence rates of married women who had leukorrhagia problem were30.69%.Leukorrhagia is common in all sorts of vaginal and cervical inflammation, involving in many diseases of modern medicine, such as:Vaginitis, cervicitis, pelvic inflammatory disease, gynecological tumors and other diseases. In recent years, its incidence is more clearly upward trend. At present, modern medicine has some effect for the leukorrhagia patients caused by genital inflammation (known as inflammatory leukorrhagia), but little effect for the leukorrhagia patients caused by non-genital inflammation (called the non-inflammatory leukorrhagia). Traditional Chinese Medicine has a long history of understanding the leukorrhagia. The TCM literatures contain a large number of valuable experiences for the treatment of leukorrhagia. Modern TCM has carried on in-depth research on etiology and pathogenesis of leukorrhagia. Under the guidance of the overall concept, we have achieved a good effect in clinical practice by carrying on treatment upon syndrome differentiation. However, the modern studies of leukorrhagia have more discussion on inflammatory leukorrhagia, but little on non-inflammatory leukorrhagia. Therefore, the study of non-inflammatory leukorrhagia is an important topic, it is worthy of further investigation in TCM. Massage as an important part of the external therapies of TCM, have been widely used in clinical, involving the treatment of tendon injuries, internal medicine, gynecology, pediatric, ENT and so on, and achieved very good curative effect. So what kind of effect will happen if we treat non-inflammatory leukorrhagia with massage? Is this method feasible? These questions need us to study and to prove.Objective:This clinical randomized controlled study adopts the clinical research methodology and statistical analysis methods, observe the clinical effect of applying Tuina to treat non-inflammatory leukorrhagia caused by spleen-deficiency syndrome. By discussing its mechanism, to determine the effectiveness of this method, so as to provide new thinking method and meaningful clinical reference for the clinical treatment of inflammatory leukorrhagia.Methods:According to the unified diagnostic criteria, we collected30cases from the gynaecology department of Affiliated Dongzhimen Hospital of Beijing University of TCM (Sept.2013-Jan. 2013) as the object of observation. All the30patients were diagnosed as non-inflammatory leukorrhagia caused by spleen-deficiency syndrome. Through the computer random grouping,30subjects were assigned to the massage treatment group and the control group,15cases in each group. The Treatment Group:Adopt massage technic treatment,3days after the end of menstruation patients began receiving treatment, daily once, five time is a course of treatment. Have a rest of two days before the second course of treatment, persists three courses of treatments. The Control Group:Adopt Wandai Decoction. Prescription: radixcodonopsitis30g, atractylodes macrocephala15g, white peony root15g, Chineseyam20g, atractylodes15g, orange peel lOg, radix bupleuri10g, black fineleafschizonepeta herb15g, isapgol15g, licorice10g. Decoct one dose with water daily and take half in the morning and half in the afternoon. Start taking it after the menstruation stops.7days make1treatment course and take for3consecutive courses. Before and after treatment, evaluate the total effective rate of the two groups of patients, valuate the clinical symptoms and body signs of the treatment group, and examine safety.Results:1. After statistical treatment, the curative and effective rate in treatment group was60%, the total effective rate was86.7%.In the control group, the curative and effective rate was80.0%, the total effective rate was93.3%. No obvious difference was found between two groups (P>0.05). Prompt that massage has the similar efficacy with Wandai Decoction, has the function of treatment of spleen deficient.2. In treatment group, before the treatment, the scores of main symptoms, minor symptoms, local clinical signs, the total clinical symptoms were9.07±2.58,5.0±2.10,3.53±1.36,17.60±5.55. after the treatment, the scores of main symptoms, minor symptoms, local clinical signs, and the total clinical symptoms were2.67±2.69,1.53±1.60,1.07±1.10,5.27±4.79. After the treatment, all scores were significantly decreased. Compared with pre-treatment, there is significant difference (P<0.01).It indicates that massage have a significant effect for improving the clinical symptoms, especially for improving the symptom of vaginal discharge volume.3. In addition, massage had a significant effect on improving patient pale complexion, poor appetite, tiredness limbs, pale mouth thirst and other symptoms.Conclusion:Studies show that massage can tonify the spleen and promote Spleen Yang, eliminate the dampness, stop vaginal discharge. It is an effective treatment for non-inflammatory leukorrhagia caused by spleen-deficiency syndrome. Massage therapy is safe and reliable, it has a significant effect, and patients are willing to accept. It provides new ideas and methods for the prevention and clinical treatment of non-inflammatory leukorrhagia caused by spleen-deficiency. So it is worthy of further research and application.
