Treatment of Chronic atrophic gastritis (CAG), the key point in process of gastric cancer, helps to prevent the occurrence of gastric cancer. There are many treatments available, Traditional Chinese medicine (TCM) has its unique advantages in treating CAG.There are several essential components in Shenqi Xiaopi Grenules (SQ), including codonopsis pilosula, lindera aggregata, lilium brownii, citrus medica, salvia miltiorrhiza, panax notoginseng, curcuma phaeocaulis,oldenlandia diffusa, bolbostemma paniculatum and taraxacum mongolicum. The previous research showed SQ could improve the secretion of mucosa nutrition factor and stimulate gastric acid secretion in CAG rats, it could also promote the repairment of mucosal, improve the function of the stomach and prevent gastric atrophy. Therefore, we observed the anti-inflammatory and analgesic effects of SQ and its effects on gastric function, the possible mechanism of SQ treating CAG rats were investigated.1. The effect of SQ on gastric functionThis part aimed at investigating the effects of SQ on gastric function. To observe the effect of SQ on mouse gastric dynamics, the model of gastric emptying disorder caused by atropine and gastric emptying hyperthyroidism caused by neostigmine in mice were applyed in this study. The model of achlorhydria caused by cimetidine in rats was established to observe the effect of SQ on gastric secretion. The study found that there was no significant difference between SQ groups and control group on gastric emptying rate (P>0.05). In the model of gastric emptying disorder and gastric emptying hyperthyroidism, the gastric emptying rate of SQ30g/kg was significantly lower than model group respectively (P<0.01). These results suggested that SQ had no distinct effect on gastric emptying in normal mice but showed synergistic effect to gastric motility disorder caused by atropine, it could produce antagonism action to gastric emptying hyperthyroidism caused by neostigmine and suggested it should be an useful inhibitor of gastric motility. In a rat model of gastric anacidity, the pH of SQ groups was obviously lower than it in the model group (P<0.05or P<0.01), the pepsin activity of SQ groups were kept at a higher level (P<0.01), but there was no significant difference on gastric barrier mucus (P>0.05). All above results indicated that SQ could emphasize acidity of gastric juice, increase proteinase activity in cimetidine gastric anacidity rat experiment.It should promote digestion.2. The study on anti-inflammatory and analgesic effect of SQ This part was to explore anti-inflammatory and analgesic effect of SQ. To observe the effect of SQ on acute inflammation, the model was built by xylene-induced ear swelling in mice, HAc-induced the permeability increase of blood capillary in mice, carrageenin-induced paw edema in rats. To analyse the effect of SQ on chronic inflammation, the model of rat cotton pellet granuloma was used. Acetic acid writhing was adopted to observe effect of SQ on analgesia. It was found that the SQ could obviously inhibit the ear swelling of mice (P<0.05or P<0.01), significantly inhibited the capillary permeability-increasing in mice belly induced by Hac (P<0.01), significantly inhibited the rate of rat paw edema at all time points (P<0.05or P<0.01). SQ9g/kg and SQ4.5g/kg were capable to inhibit cotton pellet granuloma hyperplasia significantly (P<0.05or P<0.01). SQ groups had lower writhing times than model group within20min (P<0.05or P<0.01). All these demonstrated that SQ was able to antagonism xylene-induced ear swelling in mice, HAc-induced the permeability increase of blood capillary in mice, inhibited carrageenin-induced paw edema in rats and the forming of cotton ball-shaped granuloma in rats, indicated that it had significant effects on the acute-inflammation and chronical inflammation. Its effect of peripheral analgesia was showed by lowering the writhing times of body sprain caused by acetic acid.3. The mechanism of SQ in the treatment of CAG caused by ammonia with multi-factorThis part was about the mechanism of SQ in the treatment of CAG caused by ammonia with multi-factor. The CAG rat model was established by ammonia with multi-factor by combining incitant such as ammonia alcohol compound, sodium deoxycholate, sodium salicylate and high salt diet together with irregular eating, hot and cold disorder.Epidermal growth factor (EGF), vascular endothelial growth factor (VEGF), H+-K+-ATP enzyme, interleukin prime-1β (IL-1β), tumor necrosis factor-a (TNF-a), nuclear transcription factor-κBp65(NF-κBp65) were examined by immunohistochemistry to investigate the mechanism of SQ in the treatment of CAG. The study showed that in SQ groups, the percentage of positive area of EGF, VEGF, IL-1β, TNF-α, NF-κBp65were significantly lower (P<0.01), the percentage of positive area of H+-K+-ATP enzyme was remarkably higher. These results suggested that treatment of SQ on CAG were possibly achieved by increasing secretion of gastric acid and strengthening the protection of the gastric mucosa.In summary, the certain therapeutic effect of SQ on CAG was realized probably by anti-inflammation, increasing secretion of gastric acid and strengthening the protection of the gastric mucosa.
