| BackgroundIrritable Bowel Syndrome (IBS) is a functional gastrointestinal disorder in which abdominal pain or discomfort is associated with changes in bowel habits, stool consistency and other features of disordered defecation. As a functional gastrointestinal disorder, IBS has no known organic or biochemical cause.IBS is classified as IBS with constipation (IBS-C), diarrhoea (IBS-D), Mixed IBS (IBS-M) and Unsubtyped IBS by symptoms. IBS-C may predominate. In clinical work, we found many IBS people have psychosocial problems. Because some doctors ignore the existence of psychosomatic disease, it may lead to more examination, which waste medical resources and make patients suffering and decrease the quality of their life. Currently, management of IBS includes gastrointestinal function of regulator, psychological intervention, antipsychotics, food dislodge, herbs, acupuncture and moxibustion, clysis or other symptomatic treatment. Although many drugs and methods, no drugs or managements have exact effects for all type of IBS. Therefore, combined treatment and individual treatment has been noticed by clinicians.ObjectiveAccording to the factors of IBS disease, the application of psychological intervention, Deanxit plus common drugs in the treatment of IBS-D, observation of curative effect, evaluate the therapeutic value of the scheme.MethodsThis study included108patients with a diagnosis of D-IBS in one top three hospitals of Puyang from May,2012to July,2013. According to the casual methods, it is divided into the curing group and the comparing group. Both groups were given Trimebutine Maleate, Live Combined Bacillus Subtilis and Enterococcus Faecium Enteric-coated Capsules and deanxit, psychological intervention were added to the treatment group. Psychological intervention include cognitive-behavioral intervention, emotional intervention, behavioral intervention and relaxationtraining.Results1SCL-90scores of each factors before the management of both group show many psychological problems, the major are anxiety, depress and somatization disorder, which are different from the Chinese norms (p<0.005).2SCL-90scores of each factors makes no significant differences between the two groups at the end of2,4weeks treatment (P>0.05). With the extension of treatment time, SCL-90score of the treatment group was significantly improved in anxiety, depression, somatization, compared with the control group there were significant differences after8weeks treatment (anxiety1.56±0.17vsl.81±0.36; depress1.57±0.16vs1.76±0.27; somatization disorder1.65±0.21vs1.92±0.24, P<0.05).3After treatment,96.15%good total effective rate in the treatment group was more than15.76%in the control group, that was significant statistical difference by x2-test (P<0.05). The effect of individual symptoms of the treatment group were respectively abdominal pain93.9%diarrhea96.2%abdominal distention91.7%and mucous stool92.5%, that was significantly better than the control group, which were respectively abdominal pain72.7%diarrhea88.2%abdominal distention70.3%and mucous stool65.0%. The difference between the two groups was statistically significant by x2-test (p<0.005).Conclusions:1Proper psychological intervention and deanxit had good effect on anxiety, depress and somatization disorder of people with IBS, which improved the clinical effect of treatment. 2Psychological intervention and Deanxit combining with common medicine to treat IBS-D show few side effects and more security, The maintenance dosage of Deanxit is determined, only a few have dry mouth, dizziness and somnolence, which have some worth in the application. |
