Characterized by chronic airflow limitation, Chronic obstructive pulmonarydisease (COPD) is a kind of curable and preventable disease that is not completereversible and of progressive development. COPD treatment goals are not oftenachieved though many effective treatments are applied under the traditional medicalpattern of “Physicians-Nurses Practitioners”. It is stated in “Pharmaceutical AffairsManagement of Medical Institutions” of2011that “Medical institutions shouldestablish “Physicians-Pharmacists-Nurses Practitioners”clinical treatment team toensure patients’ health, and pharmacists’ responsibility is to provide individualizedpharmaceutical care for patients. It is indicated that pharmaceutical care is of greatimportance to improve the service quality of many diseases(such as asthma, hypertension, iabetes, heart failure, etc.). However, there is no research aboutrandomized controlled trials evaluation of pharmaceutical care effects on the healthconditions of COPD patients in China. In the line with COPD diagnosed and treatment guidelines revised by Chinesemedical association respiratory neurology in2007, patients were selected fromJanuary2013to October2013in respiratory department of our hospital. According tothe COPD severity classification, patients were randomly divided into two groups.Patients in Group One were treated in “Physicians-Nurses practitioners” pattern(control group), while patients in Group Two were treated in“Physicians-Pharmacists-Nurses Practitioners” pattern(intervention group). Bymonitoring the patients’ medication adherence, incidence of ADE, patients’satisfaction, pulmonary function and the investigation for St. George’s respiratoryquestionnaire, we evaluated the impact of pharmaceutical care on the healthconditions of patients with COPD.Compared with control group, medication adherence of patients in pharmacyintervention group improved significantly (P<0.05); the incidence of ADE decreasedsignificantly (P<0.05); and the patient satisfaction with hospitalization improveddramatically (P<0.05); pulmonary functions showed no significant difference (P>0.05); the test values of SGRQ in treatment group decreased remarkably (P<0.05).The results showed that standardizing pharmaceutical care provided for COPDpatients could improve patients’ medication adherence, satisfaction and lower theincidence of ADE, improve the health conditions of patients. Complying with trend, medical institutions should establish “Physicians-Pharmacists-Nurses Practitioners”clinical treatment teams and encourage clinical pharmacists to provide individualizedpharmaceutical care for patients positively to guarantee the safety, efficiency andrational use of medication for patients.
