Clinical Research On Three-Dimensional Spine Manipulation For The Treatment Of Degenerative Lumbar Spondylolisthesis

Objective The purpose of this normalized clinical research which systematic observes and objective evaluates the effect of three dimensional spine manipulation therapy for the treatment of degenerative lumbar spondylolisthesis, is to explore its therapeutic mechanism reasonable which can provide application basis for the treatment of degenerative lumbar spondylolisthesis by manipulative massage therapy.Furthermore, This study will provide a new effective security simple method for the clinical treatment of the disease by clearing part treatment feature of three dimensional spine manipulation therapy and discussing the norm of operation which can be popularized and applied.Method Eighty spondylolisthesis patients who fit in the inclusion criteria were randomly separated into treatment group and control group, ten for each group. Patients in treatment group received three dimensional spine manipulation therapy. Patients in control group received the reduction therapy with pillows in a position of flexed cord and knees in teaching material. One course of treatment for two weeks, the treatment lasted for two courses. Japan Orthopedics Association Scores (JOA) and Visual Analogue Scale (VAS)were used to evaluate the level of pain and the clinical outcomes of patients received different massage therapy before and after treatment and after one month, the effectiveness, security and compliance of the manipulation were assessed.Results (1) After two courses of treatment, actually seventy-five cases of eighty were completed, in treatment group of thirty-eight spondylolisthesis patients who fit in the inclusion criteria, seventeen patients recovered, eleven patients had marked improvement, seven patients had effective change, and three patient had no effect. In control group of thirty-seven spondylolisthesis patients, seven patients clinical controlled, seven patients had marked improvement, twelve patients had effect change and eleven patients had no effect. The effective rate of treatment group is92.11%and control group is70.27%. The results were statistically analyzed by chi-square test, compared to the treatment group and control group in overall curative effect, there was a statistical significance (P<0.05), the overall effect of treatment group was obviously better than the control group.(2) In the aspect of clinical symptomatic improvement, not only between pre-therapy and post-treatment in each group, but also between the two groups after treatment, there were statistical differences, the former(P<0.01),the latter(p<0.01). It showed that the treatment group was obviously better than the control group.(3) In the aspect of pain’s relief, intra-group comparisons of both two groups showed that there was significant difference in before and after treatment(P<0.01); and significant differences between groups (P<0.01).It showed that the treatment group was obviously better than the control group.(4) In the aspect of improving the slippage rate, there were no statistically significant difference (P>0.05) in both intra-group and inter-group, the two kinds of methods have no obvious effect on the improvement of the slip rate.(5) The two groups were compared in the follow-up after treatment for one month, there was a statistical significance (P<0.05), the treatment group was obviously better than the control group in long time action and stability.Conclusion In the treatment of degenerative lumbar spondylolisthesis, three dimensional spine Manipulation massage had a significant effect, it brought better and quicker curative efficacy than the traditional massage therapy in the overall effect.
