Objective:To investigate the outcomes of laparoscopic surgery forendometriosis complicated with infertility and the related factors influencingthe post-treatment pregnancy. All of them have fertility requirements. All thefactors, including the age distribution, intraoperative lesions distribution,adhesion and ovarian chocolate cyst diameter, side of ovarian chocolate cystoccur, postoperative stage, may affect the postoperative pregnancy rate. Thepurpose of this paper is to study would the factors, such as age distribution,intraoperative lesions distribution, adhesion and ovarian chocolate cystdiameter, side of ovarian chocolate cyst occur, postoperative stage, affect thepregnancy rate or not.Methods:From Jan2008to Jan2013,155patients with endometriomaundergoing laparoscopic ovarian endometrioma excision in the SecondHospital of Hebei Medical University were studied retrospectively. The155patients were classified into four groups according to the stage,16r-AFSstage I cases,21stage II cases,75stage III cases,43stage IV cases. All0f the155cases, with definite diagnosisa of endometriosis, were retrospectivelyanalyzed. The retrospective analysis of the clinical and intraoperative situation,including the age distribution, intraoperative lesions distribution, adhesion andovarian chocolate cyst diameter, side of ovarian chocolate cyst occur,postoperative stage, were the main method. Recording the pregnancies of the155patients who were treated by laparoscopy in virtue of postoperativeoutpatient reexamination and telephone follow-up. By the analysis ofendometriosis infertility patients age, stage, ovarian chocolate cyst diameter,side of ovarian chocolate cyst occur, to explore their effects on pregnancy rateof postoperative pregnancy by laparoscopy.Results:All the155cases were operated by laparoscopic. The post-operational rate of pregnancy was29.03%(45/155). In women with stageⅠ-Ⅳendometriosis the pregnancy rate were57.89%(11/19),52.17%(12/23),26.09%(18/69),09.09%(04/44), rsepectively (X2=22.44,P<0.05). Thepregnancy rate of the patients with single ovarian chocolate cyst was44.23%(23/52), and that in the patients with chocolate cyst complicatde with ectopicendomtrium in other positions or those without chocolate cyst were29.27%(12/41)and16.13%(10/62), respectively(X2=10.84,P<0.05). The postoperativepregnancy rate of the patients with chocolate cyst in the right ovary [53.85%(21/39)] was significantly higher than that in the cases with left chocolate cyst[23.26%(10/43)] or chocolate cysts in both sides [5.71%(2/35)],(X2=21.93,P<0.05). The pregnancy rate of the patients with chocolate cyst ofdifferent diameters are differgent,less than3cm55.00%(11/20),3-5cm41.56%(32/77), more than5cm20.69%(12/58), X2=10.11,P<0.05)]. In different agestages the rate were20-25y53.85%(19/42),26-30y29.85%(20/67), olderthan30y13.04%(6/46), respectively(X2=11.08,P<0.05).Conclusions:Endometriosis is sure to affect a woman’s fertility,endometriosis clinical stage, and chocolate cyst occurs within different diseaselesion side and ovarian chocolate cyst diameter has an effect on pregnancyrate after treatment.Interventions shall be carried out for such patients toimprove the pregnancy rate.
