Clinical Observation Of Axial Stress Stimulate Combined With Zhuangjin Xugu Wan To Treatment Of The Comminuted Femoral Shaft Fractures:a Randomized Clinical Controlled Study

Objective:To evaluate the clinical efficacy of stress stimulator combined with Zhuangjin Xugu Wan for the treatment of comminuted femoral shaft fractures.Methods:From January2012to January2013,120patients with comminuted femoral shaft fractures were randomly divided into SS group(stress stimulator), ZXW group(Zhuangjin Xugu Wan), Combined group (SS combined with ZXW) and Control group using SPSS software according to their admission sequence. The SS group was treated with stress stimulator twice a day after2weeks postoperatively. And stress frequency was sent at0.5Hz, while stress strength was sent at250N. The ZXW group was treated with Zhuangjin Xugu Wan4.3g twice a day after2weeks postoperatively. The Combined group were treated with stress stimulator and Zhuangjin Xugu Wan, while the stess frequency and strength and the dose of ZXW was same to the SS and ZXW group. Clinical assessing including VAS pain scores, the thigh swelling, the weight bearing status of the affected leg were conducted at2week,3,6month and one year after surgery. Bone callus growth was evaluated by radiography at3,6month and one year. Fracture healing time and delayed union and non-union was also evaluated using SPSS18. software statistics.Results:All the patients were followed and mean follow-up time was12.6months(range,12-14months). None of the patients had complications. At the3months postoperatively, the VAS pain scores and thigh swelling of the ZXW group was better than SS and Control group(P<0.05), the SS group was the same as the Control group(P>0.05), and the Combined group was better than the SS and Control group(P<0.05). The growth of bone callus and the limb weight bearing of the SS, ZXW and Combined group at the3,6months and one year were significantly better than the Control group(P<0.05), the Combined group was better than SS and ZXW group, and the SS group and ZXW group had no statistical differences(P>0.05).The clinical fracture healing time of the SS, ZXW and Combined group at the3,6months and one year were significantly better than the Control group(P<0.05), the Combined group was better than SS and ZXW group, and the SS group and ZXW group had no statistical differences(P>0.05). While the delay union and un-union of the SS and Combined group were superior to Control group (P<0.001), the SS, ZXW and Combined group had no statistical difference(P>0.05)Conclusion:With the early treatment of comminuted femoral shaft fractures postoperatively using the stress stimulator combined with Zhuangjin Xugu Wan, it was effective to promote fracture healing and reduce fracture delay union and un-union. Meanwhile the axial stress combined with Zhuangjin Xugu Wan was significantly better than stress stimulator or Zhuangjin Xugu Wan alone. With using of Stress stimulator combined with Zhuangjin Xugu Wan, the VAS pain and thigh swelling were effectively reduced at3months after surgery. As a result, combined therapy may be used routinely as an adjuvant treatment for comminuted femoral shaft fractures.
