Purpose:Observe the rotating platform prosthesis and fixed platforms prosthesis two kinds of total knee prosthesis joint replacement, surgery (Total Knee Arthroplasty TKA) Recent clinical efficacy compared two total knee replacement surgery prosthesis relieve knee painreconstruction of knee function, to correct the knee joint deformity difference, provide the basis for clinical better selection of the prosthesis.Methods:Meet the diagnostic criteria,72patients (86knees) were randomly divided into the rotating platform group, fixed platform group, for total knee arthroplasty. Rotating platform group of36patients (43knees),17males (18knees) and19females (25knees), aged50years to87years, with an average age of67.34years, duration of5to30years, mean15years; fixed platform group of36patients (43knees).20males (21knees) and16females (22knees), aged53years to90years, an average of66.52years, duration of4to30years, an average of15.5years.1,3,6,12,18,24months after surgery on a regular basis to review or the phone tracking follow-up, and with the use of KSS and HSS scoring criteria for efficacy evaluation of the knee. SPSS13.0statistical software on a rotating platform with fixed platforms knee replacement in the early clinical efficacy analysis, comparison, evaluation prosthesis early postoperative efficacy differences, P<0.05as statistically significant difference between the groups, P<0.01significant difference between the two groups. And analyze a variety of related factors for total knee set curative effect.Results:Choose type rotating platform prosthesis or fixed platform type of prosthesis, postoperative good clinical curative effect can be obtained:by HSS score, KSS score between the two groups, the excellent and knee joint of motion (ROM) and complications compared to no obvious difference.(1) rotating platform group at the time of the last follow-up HSS scoring average of86.68points, was93.02%, the fixed platform group at the time of the last follow-up HSS scoring average of84.28points, was90.69%, two groups were compared with preoperative obviously improved (P<0.01).2rotating platform at the time of the last follow-up KSS clinical scoring average of86.75points, was95.34%\KSS functional score averaged87.68points, was93.02%, compared with preoperative obviously improved (P<0.01).(3) fixed platform group at the time of the last follow-up:KSS clinical scoring average of84.26points, was97.67%\KSS functional score averaged83.19points, was93.02%, compared with preoperative obviously improved (P<0.01).(4) rotating platform set is ROM the last postoperative follow-up of patiencs with an average of118.67°(P<o.01, t test). Fixed platform set is the ROM from the last postoperative follow-up of patients with an average of115.67°(P<0.01, t test).5] complications:at the time of the last follow-up in72patients,86knee, complications of total5knees, the overall incidence of complications was5.81%. Rotating platform group complications, a total of2knees, the incidence of complications was5.55%. Fixed platform set of complications, a total of3knee, the incidence of complications was8.33%. Between the two groups (P>0.05); Complication rates between the two groups had no statistical significance.[6] contrast groups themselves, whether it is a rotating platform group or fixed platform, the HSS score, KSS score, dykes and knee joint of motion (ROM), at the time of the last follow-up is all have obvious preoperatively, the differences were statistically significant (P<0.05). Once compared between two groups, the HSS scoring, KSS score, dykes, and on the knee joint of motion (ROM) had no statistical difference (P>0.05), at the time of the last follow-up rotating platform of data are slightly higher than the fixed platform group, after t test found that the rotating platform and fixed platform group differences had no statistical significance (P>0.05). Group (8) rotating platform for false body and in-hospital total cost ratio fixed platform group’s costs and in-hospital total fee is high. The above results show that:the rotating platform type artificial prosthetic or artificial prosthesis fixed platform, two kinds of prosthesis can achieve good clinical curative effect of early postoperative; Through the HSS score between the two groups, KSS score, fine rate of motion (ROM) of knee joint and contrast found that rotating platform of artificial prosthesis with artificial prosthesis fixed platform group was no significant difference between the two groups.Conclusion:Patients in72cases (86knees) research shows that: the rotating platform prosthesis and fixed platform prosthesis surgery good clinical curative effect can be obtained. Fixed platform and rotary platform group between patients in the preoperative and postoperative follow-up sessions at the end of the HSS score, KSS score, dykes, and on the knee joint of motion (ROM) had no statistical difference (P>0.05), at the time of the last follow-up rotating platform of data are slightly higher than the fixed platform group, after t test found that the rotating platform and fixed platform group differences had no statistical significance (P>0.05). Rotating platform and fixed platform group compared with no significant difference. The early curative effect are satisfied, the long-term curative effect is longer observation should be carried out in more patients. Therefore, prosthesis selection is not the important factors that affect knee place early postoperative curative effect.
