Comparative Of Mid-term Clinical Efficacy About Rotating-platform And Fixed-platform Prosthesis In Total Knee Arthroplasty

Objective: To compare and analyze the mid-term effect of mobile-bearing prosthesis(MB)and fixed-bearing prosthesis(FB)in total knee arthroplasty(TKA),and to provide the reference for clinical selection of prosthesis.Methods: From March 2013 to March 2015,50 patient(61 knees)undergoing total knee arthroplasty with fixed-platform prosthesis(Smith-Nephew Genesis II)and rotating-platform prosthesis(Zimmer LPS-Flex Mobile Bearing System)in First Affiliated Hospital to Soochow University were analyzed retrospectively.They are divided into two groups:18 patients(24 knees)with fixed-platform group and 32 patient(37 knees)with rotating-platform group.The Hospital of Special Surgery(HSS),KSS knee joint score,KSS function score,knee movement(Range of motion,ROM)and imaging data were compared between the two groups at the time of preoperative and the last follow-up.SPSS22.0 software was used to statistically analyze the data.P < 0.05 was considered statistically significant.Results: The two group patients' average length of follow-up was 4.1years.There were no statistical differences in age,sex,constitution ratio of disease,preoperative HSS score,preoperative KSS score and preoperative ROM in the two groups(P>0.05).At the last follow-up,the HSS scores of the fixed-platform group was 91.11±3.81,KSS knee score was 90.71±4.72,KSS functional score was 90.21±8.14,ROM was 118.54±9.15;the HSS score ofthe rotating-platform group was 90.65±4.44,KSS knee score was 91.54±3.87,KSS function score was 90.54±8.48,and ROM was 122.3±10.65.At the last follow-up,the HSS score,the KSS knee score,the KSS function score and the ROM of the two group were higher than those before the operation.Both the fixed-platform group and the rotating-platform were statistically significant(P<0.05).There was no statistical difference between the two groups(P>0.05).The two groups' X-ray showed that the prosthesis was well positioned,and periprosthetic osteolysis,X-ray bright line and prosthesis loosening were not found.So far,no revision is needed due to loosening of prosthesis or other reasons.Conclusion: There is no statistical difference between rotating-platform prosthesis and fixed-platform prosthesis in the knee scores,the range of motion and the radiographic results,and each prosthesis can achieve a satisfactory medium-term clinical effect.
