Construction Of Health And Epidemic Prevention System Of UN Peacekeeping Chinese Level Ⅱ Hospital In Lebanon

Objective：United nations (UN) peacekeeping Chinese level Ⅱ hospital in Lebanon is oneforeign-related medical institution to provide medical services to united nations interim forcein Lebanon (UNIFIL), the main responsibility is to provide health care for the members ofmilitary contingents, military observers, civilian police, international staff members, and UNcontracted staff members in east UNIFIL, it also provide humanitarian aid to local population.Except for peacekeeping medical missions, UN also appoints one hygiene officerattached to level Ⅱ hospital to carry out the health and epidemic prevention work of its ownand east UNIFIL.Reasonable health and epidemic prevention system construction andpractice will directly affect the implementation of the health and epidemic prevention efforts,it is the key to whether Chinese level Ⅱ hospital can complete peacekeeping missionseffectively, not only involving all the institutions and personnel of hospital, but throughout thewhole peacekeeping mission process, has very vital significance.This study analyzed the component contents of health and epidemic prevention system inlevel Ⅱ hospital systematically proposed by the practical experience of the author whoparticipated in the health and epidemic prevention work of UN peacekeeping Chinese levelⅡ hospital in Lebanon as hygiene officer, in addition, combined with our realities anddemands of UN, to explore and establish the feasible health and epidemic prevention supportmechanism, so as to provide scientific basis for efficient implementation of the health andepidemic prevention work in UN peacekeeping Chinese level Ⅱ hospital.Method：1.Investigation on the current situation of health and epidemic prevention system ofprevious batch UN peacekeeping Chinese level Ⅱ hospital in Lebanon The sixth batch UN peacekeeping Chinese level Ⅱ hospital in Lebanon from July2010to February2011was taken as object, the investigation and studies were carried out in theaspects of organizational system, personnel, equipment, disease surveillance, healthsupervision, disinfection, insecticide, deratization, health protection, health education,veterinary health, health information, and the completed tasks related to UN health andepidemic prevention, analyzed the current situation of their health and epidemic preventionwork, so as to summarize experience and deficiencies.2.Preliminary construction of the new health and epidemic prevention system of UNpeacekeeping Chinese level Ⅱ hospital in LebanonThe new health and epidemic prevention system was preliminary constructed on thebasis of UN memorandum of understanding and standard operating procedures (SOP), andtook passing various UN inspections as the criterion during task period of the seventh batchUN peacekeeping Chinese level Ⅱ hospital in Lebanon from February2011to October2011,by resolving the shortage problems of health and epidemic prevention work of previous batchlevel Ⅱ hospital, and combining with the current specific contents of health and epidemicprevention work, such as task source, characteristics, structure, equipments, implementationand so on.3.Application and verification of the new health and epidemic prevention systemTo command health and epidemic prevention work by the new system, to apply andverify the system in the actual operation, and to evaluate the work efficiency, so as to putforward improvement plan during task periods of the eighth and ninth batch UN peacekeepingChinese level Ⅱ hospital in Lebanon from October2011to March2013.Results：The survey of current situation of health and epidemic prevention system of previousbatch UN peacekeeping Chinese level Ⅱ hospital in Lebanon showed that:(1) Though itcould carry out the health and epidemic prevention work basically according to the UNrelevant requirements, it didn’t grant hygiene officer to match his power authority on thesystem of organization.(2) Shortage of health and epidemic prevention staff, aging of healthand epidemic prevention equipments, and the weak ability of self sustaining.(3) Diseasesurveillance, health supervision, disinfection, insecticide, and deratization were still the coreof health and epidemic prevention work in level Ⅱ hospital.(4) The negative factors such as lagging concept of health and epidemic prevention, backward hardware and facilities in thecamp, sharing the food refrigerated container and the morgue, and could not maintain theincinerator timely were the key links that constrained the health and epidemic preventionwork of level Ⅱ hospital.(5) Because of the substandard results as required of varioushygiene inspections that carried out by UN, the subsequent corrective pressure was great.(6)The new scientific and reasonable, close to the actual, and highly operable health andepidemic prevention system was badly in need of research.The new health and epidemic prevention system of UN peacekeeping Chinese level Ⅱhospital in Lebanon had been preliminary constructed, with the focus on solving the followingproblems:(1) Standardize the organizational system and staff composition of health andepidemic prevention work.(2) Provided the corresponding equipments to peacekeepingmission.(3) The SOP and related rules and regulations of health and epidemic preventionwork had been established by combining with the realities of level Ⅱ hospital, to ensure thevarious health and epidemic prevention work to implement effectively.(4) It could coped withthe health and epidemic prevention work effectively in the key parts such as operating room,dining room, material container, morgue, incinerator, and the key links such as HIV(AIDS)infected prevention, medium of harmful organisms prevention, medical waste disposal,nosocomial infection control and prevention, and the dead bodies disinfection.(5) To makesure that peacekeeping medical contingent passed the self-sustaining equipment inspection,food inspection and hygiene inspection according to the regulations of UN.3.The popularization and application results of the system showed that both the eighthand ninth batch UN peacekeeping Chinese level Ⅱ hospital in Lebanon completed healthand epidemic prevention work efficiently, and there were no communicable disease cases, nonosocomial infection, and no food poisoning events happened in the task period, meanwhile,they passed through the various UN inspections smoothly, and properly handled the healthand epidemic prevention problems of adjacent troops in the east UNIFIL.But there is stillneed to improve the aspects mainly as follows:(1) It should strive to resolve problem aboutthe weak continuity of health and epidemic prevention work due to logistic supportdifficulties in level Ⅱ hospital.(2) It should pay attention to integrate the health andepidemic prevention system and the medical service.(3) It should establish the SOP onrotation and hand-over of health and epidemic prevention work.(4) The operating methods of health and epidemic prevention work in the case of emergency should be set up in the system.
