Objective:To study the current treatment status of COPD patients through questionnaire survey, regular follow-up, monitoring of patients’ condition and the dynamic changes of pulmonary function, and respiratory exercise teaching. Compare matched group with the control group to understand whether patients who do breathing exercises can alleviate the symptom, improve patients’quality of life and improve the pulmonary function.Method:Select randomly100COPD patients who came to respiratory department of our hospital from January to June in2012, and conduct questionnaire, pulmonary function test on all patients.86patients of them are randomly assigned to matched group and control group, and teach the control group of42patients breathing exercise exactly. Perform follow-up via telephone on the control group for six months. Pay a return visit to all patients after6months, and do the survey questionnaire and pulmonary function again, and to collate and analyze the collected data.Results:There were86male and14female in the100patients with COPD. Their average age was (65.98±8.81) years old, of whom there were33people have exposure history of occupational dust and smog, and31have related respiratory diseases in the family. The average smoking index was (43.21±34.85) pack year. Over the past12months, the median of AECOPD in the100patients was1.The median of medical costs in AECOPD was800yuan. The average score of understanding level was (4.15±2.24) points. There were positive correlations between understanding level and level of education and income of the patients. The average index of body mass index (BMI) was (21.33±4.19) kg/m2. The mine nutritional assessment (MNA) score showed that44patients have malnutrition risk,10patients have self-rating depression acale(SDS) score show that8have mild depression,1have severe depression. The average CAT score was (19.22±7.22). There are positive correlations between FEV1%pred and BMI. There are negative correlation between FEV1%pred and those indexes, such as6MWT, SDS score, CAT score, mMRC rating, smoking index, and the number of acute attack. After6month with rehabilitation training, there are also statistically differences between the control group and intervention group in the difference of the FEV1%pre (F=0.382, P=0.002), mMRC rating (F=1.128.P=0.016), CAT score (F=1.127.P=0.012) and6minutes walking distance (F=1.109.P=0.009).Conclusion:1. In the group of COPD patients, the proportion of low-income people, low-education people and smoker is high. The disease impacts on patients’ nutrition, quality of life and psychology.2. COPD patients have a low awareness of their own poor disease. There were positive correlations between understanding level and level of education and income of the patients.3. There are negative correlation between the level of lung function impairment and the quality of life, nutrition; and positive correlations between the level of lung function impairment and the severity of dyspnea, depression and anxiety.4. Breathing exercises help to alleviate patient symptoms of dyspnea, improve the life quality of patients, and slow their lung function down.
