| Objectives: This study aims to compare the effects of traditional Chinese and Western medicine respiratory rehabilitation on the respiratory function and quality of life of patients with COPD with pulmonary spleen-qi deficiency and to evaluate the clinical efficacy and value of TCM rehabilitation training for patients with stable COPD,with a view to clinical disease Rehabilitation treatment provides ideas.Methods: In this study,patients with stable COPD with lung and spleen qi deficiency and pulmonary function grades II and III were used as the research object.A randomized controlled clinical study was used to enroll in the first affiliate of Tianjin University of TCM from October 2019 to October 2020.There were 96 patients who met the standards in the outpatient department of the hospital’s respiratory medicine department.They were divided into a TCM rehabilitation group,a Western medicine rehabilitation group and a control group,each with 32 cases.On the basis of the original standard treatment and health education of western medicine,all patients do not require additional drug intervention.Patients in the TCM rehabilitation group take "Strengthen six-character training" that increase the number of times of "hu" and "si" tactics as the main training content.Western medicine recovers The main training content was breathing training in the group,and no other intervention in the control group.After passing the training,the patients will undergo 12-week pulmonary rehabilitation exercises,go to the hospital for intensive training once a week,and complete the rest of the time at home,and complete the exercise log and telephone follow-up in time.Finally,the lung function(FEV1%,FEV1/FVC),CAT score,m MRC score,TCM syndrome score,etc.as the main efficacy observation indicators.The data were analyzed by SPSS23,and(P<0.05)was considered as statistically different,(P<0.01)as significant difference,and(P>0.05)as non-statistical difference.Results: A total of 96 patients were enrolled in this study,who failed to complete the experiment due to loss to follow-up or poor compliance.There were 2 patients in each group.There were 90 effective cases,30 in each group.1.Baseline data: general conditions of the three groups of patients before treatment and initial efficacy indicators(pulmonary function,CAT score,m MRC score,TCM syndrome score)There was no statistical difference(P>0.05),indicating comparability.2.Comparison of CAT scores: After treatment,the CAT scores of the Chinese and Western medicine rehabilitation groups were significantly lower than before(P<0.01);in comparison between the groups,the improvement of TCM rehabilitation group was powerful than that of the other two groups(P<0.05),and there was no statistical difference between the other two groups(P>0.05).3.m MRC score: The m MRC scores of the three groups decreased after treatment(P<0.05);comparison between the groups,the improvement of the TCM rehabilitation group and western medicine rehabilitation group was notably greater than the control group(P<0.01),but this two groups was no statistical difference(P>0.05).4.Pulmonary function: After treatment,the FEV1% value of the three groups of patients showed a downward trend,the FEV1% value of the control group decreased notably(P<0.05),and there was no statistically significant decrease in the value of the TCM and Western medicine rehabilitation groups(P>0.05).In the comparison between the groups,the drop difference in the TCM rehabilitation group was lower than that in the control group(P=0.021<0.05),and there was no statistical difference between the other two pairs(P>0.05).From the value of FEV1% before and after treatment,the decline in TCM rehabilitation group was the smallest,and the rate of decline in lung function was the slowest.After treatment,there was no statistical difference in FEV1/FVC among the three groups(P>0.05),and there was no statistical difference in the degree of improvement between the groups(P>0.05).5.TCM syndrome score: Three groups of patients after treatment,TCM Syndrome Score significantly improved(P<0.01);comparison between the groups,the improvement of the TCM rehabilitation group was notably better than other two groups(P<0.01).The improvement of the western medicine rehabilitation group was notably greater than that of the control group(P<0.01).6.Symptoms of TCM sub-items: After treatment,the symptoms of cough,expectoration,wheezing,and shortness of breath were significantly improved in the three groups(P<0.01).In comparison between the groups,the improvement in the rehabilitation group of Chinese and Western medicine was better than that of the control group(P<0.05).Among the symptoms of cough and expectoration,the TCM rehabilitation group is better than the Western medicine rehabilitation group(P<0.05);the TCM rehabilitation group has obvious improvement in the symptoms of fatigue,appetite,spontaneous sweating and loose stools after treatment(P<0.01),the western medicine rehabilitation group improved in fatigue and anorexia symptoms(P<0.05),In comparison between the groups,in terms of fatigue,appetite,spontaneous sweating,and loose stools,the improvement of the TCM rehabilitation group was better than that of the Western medicine rehabilitation group and the control group(P<0.05),and there was no significant difference between the Western medicine rehabilitation group and the control group(P>0.05).After treatment,three groups in clinical effectiveness respectively is 86.67%,46.67%,3.33%.The curative effect of the two rehabilitation group was notably powerful than that of the control group(P<0.01),and the TCM rehabilitation group was powerful than the western medicine rehabilitation group(P<0.05),the effect is the best.Conclusions:1.Strengthen six-character training is better than breathing training in improving the respiratory function and quality of life of patients with COPD with deficiency of lung and spleen Qi in stable stage,and has a more comprehensive and comprehensive clinical value.2.Pulmonary rehabilitation training of TCM for patients with stable COPD can not only give full play to the unique advantages of traditional Chinese medicine,but also help improve patients’ clinical symptoms and delay disease progression. |
