Background: Severe acute pancreatitis (SAP) is a dangerous disease with highhospitalization expenses, and its mortality rate is about10%-30%. While there is nostandard guideline for its management, in this study we aimed to analysis differenttreatment efficacy of patients with SAP based on Balthazar CT grading.Method: A cohort was established with60SAP patients enrolled from the FirstAffiliated Hospital of Dalian Medical University between March2006to December2012. Data on demographic factors, laboratory test results and CT results were collectedfor patients meeting the inclusion criteria. Frequency comparisons were made with thechi-square test. Two-group comparisons of normally distributed data were performedwith the independent samples t-test. For multi-group comparisons, one-way analysis ofvariance (ANOVA) with the least-squares difference post hoc test was applied. For datanot normally distributed, the Mann-Whitney U test was used if only two groups werecompared, and Kruskal-Wallis one-way ANOVA was used if more than two groups werebeing compared.Results: In patients under Balthazar CT grade E, the relief time of abdominal pain ofsingle Chinese traditional medicine (TCM) treatment, single glucocorticoid treatmentand combined with TCM and glucocorticoid were8.9±7.5,12.0±8.7,8.7±4.0days; dietback to normal time were10.7±8.1,18.3±14.4,14.9±11.4days; length of hospital staywere16.2±7.5,28.0±19.5,19.9±13.2days; cure rates were80.0%,100.0%,66.7%,respectively. There were not statistically significant differences in the relief time ofabdominal pain, diet back to normal time, length of hospital stay, and cure rate (P>0.05)among these three groups. However, as to patients with Balthazar CT grade E, the relieftime of abdominal pain, diet back to normal time, length of hospital stay of combinedwith TCM and glucocorticoid was significantly shorter and cure rate was higher thanmonotherapy with TCM or glucocorticoid (relief time of abdominal pain were 20.8±12.0,19.3±6.7,12.9±8.0days; diet back to normal time were25.6±7.5,21.3±10.5,15.3±7.7days; length of hospital stay were21.3±8.8,31.5±13.0,35.8±14.1days;cure rates were82.4%,66.7%,40.0%, respectively, P <0.05).Conclusion: No difference was noted in the relief time of abdominal pain, diet back tonormal time, length of hospital stay, and cure rate of single Chinese traditional medicine(TCM) treatment, single glucocorticoid treatment and combined with TCM andglucocorticoid among patients under Balthazar CT grade E. While patients withBalthazar CT grade E, the relief time of abdominal pain, diet back to normal time,length of hospital stay of combined with TCM and glucocorticoid was significantlyshorter and cure rate was higher than monotherapy with TCM or glucocorticoid. |
