Objective:By analyzing the clinical characteristics of primary gastric malignantlymphoma in order to improve the understanding of the disease, in order to earlydiagnosis, early treatment, improve the prognosis.Methods:In our hospital were retrospectively analyzed the clinical data of33casesof primary gastric malignant lymphoma patients.Results:Our hospital from January2001to October2001were admitted bypathological examination after endoscopic biopsy or surgical pathology proved cases ofprimary gastric malignant lymphoma, a total of33cases,Male,14cases,female in19,male and female was in the ratio of1:1.36,age range for21-85,average age was60years old.For more than25cases50, accounting for75.8%.This group of patients abovedigestive tract symptoms of abdominal pain and epigastric discomfort (28cases,84.8%),followed by abdominal distension, loss of appetite, hematemesis black, nausea,vomiting and acid regurgitation.The endoscopic biopsy diagnosed lymphoma in29cases (87.9%), the rest4cases of surgical pathology diagnosis, pathological changes isgiven priority to with gastric antrum and the gastric body, involving the gastric antrumin5cases (15.2%),7cases (21.2%), gastric body involving sinus10cases (30.3%) ofthe body, the bottom body in7cases (21.2%), involving the two parts above4cases(12.1%).Endoscopic performance for ulcer type18cases (54.6%), and diffuseinfiltrating type7cases (21.2%); Nodular type8cases (24.2%).27patients test positivefor Hp (27/33,81.8%) of gastric mucosa associated lymphoid tissue of Hp positive16cases of malignant lymphoma (16/18,88.9%),8cases of diffuse large B cell lymphoma(8/12,66.7%). Gastric mucosa associated lymphoid tissue malignant lymphoma Hppositive rate compared with diffuse large B cell lymphoma was higher, difference havestatistical significance (P=0.036).Malignant gastric mucosa associated lymphoid tissuelymphoma (MALT lymphoma) treated18cases (54.5%), diffuse large B cell lymphomain12cases (36.4%), the merger of MALT lymphoma treated diffuse large B cell lymphoma2cases, the T cell lymphoma in1case, Clinical pathologic staging: ⅠE12cases (36.4%), ⅡE period of8cases (24.2%), ⅢE period of10cases (30.3%), ⅣEperiod3cases (9.1%).Comparison with other groups of MALT lymphoma treated group,clinical pathologic staging in MALT lymphoma treated group with ⅠE period morefrequently in patients, the results have statistical significance (P=0.025).Treated30cases,8of whom underwent surgery and postoperative adjuvant chemotherapy, death in2cases,4cases of simple surgery. The rest of the18patients received chemotherapyalone,7cases died.Conclusions:1.The helicobacter pylori in gastric MALT lymphoma treated playsan important role;2.The diagnosis of PGML requires endoscopic biopsy or surgicalremoval of tissue pathological diagnosis, the endoscopic biopsy diagnosis rate is stilllow, consider and gastroscope under improper materials cannot achieve lesions or tooshallow, so in patients with suspected the disease suggested that multiple endoscopicbiopsy and parts more deep biopsy in order to improve the diagnostic accuracy.
