Current Status Of Supervision And Management Of Clinics And Strategies Development In Kuancheng District

With the gradually established of socialist market economic systemin our country, the great changes have taken place in medical servicemarket’s scale, characteristics, scope and influence, so did the regulatorysystem, which is corresponding to the medical service market. Eventhough the health supervision system has been or is being reformed inrecent years, medical health supervision work is still a weakness.Kuancheng district is the first county、district-level administrativedistrict in Changchun city, which is the typical representative amongthose administrative district in large and medium-sized cities in the northpart of China. The total population is377,000,244clinics medicalinstitutions, which are a large share of medical institutions market inKuancheng district.Purpose:Do the investigation of the health supervision work in2012for theclinics medical institutions in Kuancheng district, in order to get a goodunderstanding of the clinics medical institutions during this period and insuch environment. Besides, do the further analysis and find out acountermeasure and focus for the next step, improving the quality ofhealth supervision work.Methods:By consulting literature materials, understand the current situation ofsupervision and administration work in clinics medical institutions bothin China and abroad. Combining with the relevant literature, we made”The Questionnaire of Medical Institution”. We investigated all the clinicsmedical institutions in Kuancheng district via on-site supervision, in orderto know whether the medical institutions abide by laws and regulations in those aspects, such as medical services, epidemic reports, medical wastetreatment, medical prevention and healthcare and so on. Do acomprehensive analysis on supervision files of clinics medical institutionsduring2009-2012, finding out problems during practicing andsupervision works.Results:1. The ratio of clinics classification is disparity.There are244registered clinics medical institutions in Kuanchengdistrict. If divided by diagnosis subjects,218of them are internalmedicine, which accounts for89.34%of overall institutions.2. Not hanging plaque of medical institutionsThere are239institutions hanging plaque, some institutions don’thang plaque.3. Staffs lack of law awareness and self-disciplinedBy the investigation, we found that:47institutions don’s have healthtechnical staffs;24institutions employed unregistered health technicalstaffs;49institutions beyond their practice;38institutions didn’timplemented “Three Rooms”.Among244instituions,85.66%of themknow the laws and regulations.4.Patients registering and prescription management are not strict.There are188institutions’ registration meet the standard;12institutions don’t have clinic register book;8institutions don’t use formalclinic register book;36institutions’ register book don’t write clearly andcompletely. Among214institutions, which need to prescribe the drugs,179institutions meet the standard.5. Infectious disease prevention and control and consciousness of hospitalinfection control need to be improved. There is no record of infection patients on register book in eachinstitution. The system of infectious diseases prevention and controlbecomes formalistic. We investigated6aspects as follows: whether theinstitutions set up disinfection and infection control rules; whether theyuse the register book of disinfection; whether the staffs have a goodcommand of disinfection process; whether the disinfection equipmentswork well; whether they can do the hands disinfecting; disinfecting itemsidentifying and disinfecting date. There are problems for all of theseparts.6. The training for medical staff is not enoughThe frequency for the training organized by relative department eachyear is not much and monotony in content, so the medical staff are hardto get comprehensive training.7. Although the strength of health administration supervision is enough,the scale of supervision still needs to improve.Between2009and2012, there is an increase in both files numberand penalty amount, which indicating the strength of healthadministration supervision has improved. But the supervision onmicrobiological test and comments on prescription for some individualinstitutions are still need to be perfect.8Local governments attach importance to the management of medicalwaste.The medical waste management office of Kuancheng district set upspecial-staff to transport the medical waste for all the medical institutionsin the district, in order to prevent medical waste flowing, which has highvalue worthy of promotion.Conclusion:In this paper, the authors present that the government shall make overall plans and check strictly, and put forward correspondingcountermeasure and suggestion on improving the standard and conditionsfor registered clinics medical institutions；providing more training andpropaganda of laws and regulations for health technical staff； enhancingthe awareness of prevention on infectious disease； strengthen thesupervision for medical institutions and improving the quality of healthsupervision group and so on. By implementation of the above opinions,improve the quality of the medical health service, strengthen the medicalhealth supervision, and improve the service environment of medicalinstitutions. Basically protect people’s health, safeguard the interests ofthe people.
