| Objective:Detect soluble myeloid cells triggered receptor-1(sTREM-1)ã€PCTã€CRP inperipheral blood of patients with pulmonary infection, observe diagnosticsignificance of sTREM-1in pulmonary infection patients, find the differencebetween CAP and HAP and evaluate its clinical application value.Methods:Use ELISA to detect sTREM-1ã€PCTã€CRP level in peripheral blood inexperimental group(including30cases of patients with CAP and18cases of HAPpatients) and control group(including14cases with diffuse pulmonary disease).Record expression of each index, and calculate sensitivityã€specificity and accuracyof single detection or combined detection.Results:(1) Compare to the diffuse pulmonary disease group, the CAP group serumsTREM-1[(99.71±111.17)pg/ml]ã€PCT[(10.26±22.02)ug/ml] are much higher(P<0.05) than the control group;(2) The HAP group serum sTREM-1[(117.40±134.73)pg/ml]ã€PCT[(8.67±9.11)ug/ml] are much higher(P<0.05) than that in thediffuse pulmonary disease group;(3) There is no significant difference in sTREM-1ã€PCTã€CRP between CAP and HAP group;(4) By the use of the ROC curve, the bestthreshold of serum sTREM-1is38.13pg/ml, in CAP group sensitivity is0.900andaccuracy is0.864, which are higher than PCT and CRP (P<0.05); in HAP groupsensitivity is0.889and accuracy is0.844, which are higher than PCT and CRP(P<0.05); specificity is0.786,no significant difference from PCT;(5) Both in CAP groupand HAP group the sensitivityã€specificity and accuracy of diagnosis of sTREM-1+PCT are higher than single detection (P<0.05).Conclusion:1. Serum sTREM-1ã€PCT and CRP level increase significantly both in CAP andHAP, the sensitivityã€specificity and accuracy of sTREM-1and PCT are better thanCRP, the sensitivity and accuracy of sTREM-1are better than PCT; 2. The combined detection of sTREM-1and PCT can improve the sensitivityã€specificity and accuracy in the diagnosis of CAP and HAPï¼›3. There is no significant difference of serum sTREM-1ã€PCT and CRP levelboth in CAP group and HAP group. |
