Objective: Determine the expression levels of the sTREM-1in BALF、serum andlungtissue,which were taken from klebsiellar pneumonia mice and treatmented mice,comparing with the normal mice to discuss the meaning of sTREM-1in diagnosingpneumonia as a inflammatory marker.Methods: Mice selected randomly were divided into there groups, control group、pneumonia group and the treatment group.Each group was20.Klebsiellar was takenfrom clinical infection patients’ sputum culture, identified by the VITEK-2system, andit was used to establish the mice pneumonia model,while control group wasadministered with an equal volume of saline. Cefotaxime was used to treating themice,and the dose was calculated according the equivalent amount of drug betweenadult and mouse.They were treatmented for three days. The lungs did the pathologicaland histological specimens observation. IL-6gene expressing in lung tissue wasdetected using RT-PCR method. Levels of sTREM-1in BALF、serum and the lungtissue were detected with ELISA method and the expressing level of CRP in serum wasalso detected with ELISA method.The results were analysised by SPSS15.0.Results: The lungs of the control group were normal in the gross appearance, while the lungs of the pneumonia group were markedly hemorrhagic and congested.The Standardization integral absorbency value(NII) of IL-6in pneumoniagroup was significantly higher compared with the control group. The treatment group comparing with pneumonia group declined obviously, and there was statistically significant (P<0.05).Bacteria cultured from pneumonia group was same as the infecting strains, and all for pure training.The expressing levels of the sTREM-1in BALF,serum and the lung tissue of pneumonia group,which were420.48±57.99pg/ml,2372.17±229.00pg/ml,522.81±56.24pg/ml,increased signifycantly than con trol group,which were34.69±2.92pg/ml,296.52±28.63pg/ml,45.98±3.17pg/ml.Andthere was statistically significant (P<0.05);The treatmented group,which were210.73±58.47pg/ml,694.30±116.19pg/ml,188.49±59.22,was decreasing obviously,but still higher than those in the control group, there was statistically significant (P<0.05).There was correlation obviously between serum sTREM-1and CRP in pneumoniagroup and the treatment group.sTREM-1increasesd with the increasing of the CRP(P<0.05), and it decreased with the increasing of the CRP(P<0.05).The diagnostic capabilities of the sTREM-1、CRP and the combined diagnosis wascalaulate applying ROC curves. ROC curves (AUC) area of the serum sTREM-1was0.886, CRP0.822,sTREM-1combining CRP0.938. when the level of serumsTREM915pg/ml, the diagnosis sensitivity for pneumonia was90%, the specific degrees65%, youden’s index0.55; When the level of serum CRP2.82ng/ml, the diagnosissensitivity for pneumonia was85%, the specific degrees75%, youden’s index0.6;sTREM-1combining CRP diagnosis of pneumonia sensitivity was95%, the specificdegrees80%, youden’s index0.75.Conclusion: The levels of sTREM-1in BALF、serum and lung tissue risedsignificantly when mice infected by klebsiella,and both of them declined significantlyafter the mice treatmented. It pointed out that there was certain reference value foruseing sTREM-1to diagnose bacterial pneumonia.
