Objective:Chronic obstructive pulmonary disease (COPD) is characterized byairflow limitation chronic airway inflammatory diseases. Pathogenesis ofCOPD include: inflammation, the imbalance of protease and antiprotease andoxidative etc. In a variety of mechaniams inflammation is the central link ofthe pathogenesis of COPD.A variety of inflammatory cells、cytokines、chemokines and adhesion molecules take part in the occurrence anddevelopment of COPD. The neutrophil activation and aggregation is animportant part of the process of inflammation of COPD. Soluble triggeringreceptor expressed on myeloid cells-1(sTREM-1) was an inflammatorycytokines discovered recently. It was expressed on neutrophils and monocytes.LeukotrieneB4(LTB4)is produced by epithelial cells、 neutrophils andmacrophages, and airway inflammation have contact closely. SummarysTREM-1、LTB4and neutrophil are closely linked. The aim was to test theconcentration of sTREM-1and LTB4in peripheral blood plasma fro frompatients with different groups of COPD, and to analysis the correlation ofsTREM-1、LTB4、the count of neutrophil and lung function. and explore theimplication for the assessment of the inflammation of COPD.Methods:First, outpatient and inpatient were informed consent, and thencompleted the pulmonary function testing. According to pulmonary functiontesting (FEV1/FVC%、FEV1predicted%)、symptom(mMRC dyspnea index)、 the number of acute exacerbations last year were completed a comprehensiveassessment. According the Global strategy for the diagnosis, management,and prevention of chronic obstructive pulmonary disease (Revised2011) thepatients were divided into four groups of A group (low-risk,less symptomsgroup)、B group (low risk, more symptoms group)、C group (high risk, lesssymptoms) and D group (high risk, more symptoms group).The patients ineach group18cases, for the healthy subjects16cases as the healthy controls.The peripheral blood of patients and healthy controls were collected, and thenmeasured the concentration of sTREM-1and LTB4by ELISA method. Thesame specimens were counted the number of neutrophils. To contrast theconcentration of sTREM-1and LTB4that was between the different groups ofCOPD. The correlation of the concentration of sTREM-1and LTB4、FEV1predicted%and the number of neutrophils was analyzed.Results:①The concentration of sTREM-1from the COPD patients ofA、B、C、D groups and healthy control groups were respectively as2.41±0.68ng/ml、4.33±0.83ng/ml、5.23±0.88ng/ml、8.44±0.69ng/ml and1.11±0.20ng/ml. Acontrast the concentration of sTREM-1from the COPD patients each groupand the healthy control group, a significant difference between each group ofCOPD and healthy control (P<0.01).②The concentration of sTREM-1from the COPD patients ofA、B、C、D groups and healthy control were negative correlated with FEV1%predicted(r=-0.675, P<0.01), and was positive correlated with the number ofneutrophils(r=0.731, P<0.01).③The concentration of LTB4from the COPD patients of A、B、C、D groups and healthy control groups were respectively as1.68±0.46ng/ml、1.96±0.12ng/ml、2.24±0.98ng/ml、2.52±0.15ng/ml and1.36±0.19ng/ml. Acontrast the concentration of LTB4from the COPD patients each group andthe healthy control group, a significant difference between each group ofCOPD and healthy control (P<0.01).④The concentration of LTB4from the COPD patients of A、B、C、Dgroups and healthy control were negative correlated with FEV1%predicted(r=-0.848, P<0.01), and was positive correlated with the number ofneutrophils(r=0.868,P<0.01).⑤The correlation of the concentration of sTREM-1and LTB4waspositive(r=0.872, P<0.01).Conclusion:①Derived neutrophil sTREM-1and LTB4involve in lung functiondamage;②As the disease in patients with COPD exacerbation the concentrationof sTREM-1and LTB4increased gradually. They may become biomarker andassess the disease among different groups of COPD;③The concentration of sTREM-1and LTB4is positive correlationamong different groups of COPD both may be on the same signaling pathwayand promote inflammation mutually.
