Objective: The topic by observing the diagnosis and treatment of children with oldMonteggia fracture in recent years, summarizes the children’s old monteggia fracture, faultdiagnosis and misdiagnosis and improper handling. Make clinical physicians emphasis onthe physical examination of the child’s elbow, seriously read the X-ray film, appropriaterepositioning maneuver, reasonable fixed position, fixed good, timely outpatient care inorder to better guide the early diagnosis and treatment of Monteggia fractureand, so as tolay the foundation for further in-depth research.Methods: The initial screening qualified cases of25patients, for a history of detailedconsultation, the treatment process and to examine, analyze the cause of children’s oldmonteggia fracture. In which5cases were misdiagnosed as soft tissue injuries,misdiagnosed as ulna or radius and ulna fracture in10cases; diagnose clearly, inadequategimmick reset6cases, the plaster loose shift two cases, bad shape or improper position in2cases. Boyd incision to expose the ulna, the humeral head and dislocation of the radialhead, proximal ulna osteotomy, radial head after reset with Kirschner wire fixation; Correctulnar angulation and shortening deformities, plate fixation. After X-ray film review.Followed up time of9-24months, postoperative X-ray review every2weeks, mainlyobserved in children with preoperative and postoperative elbow, forearm rotation, forearmmuscle strength and grip strength, etc.,comparing preoperative and postoperative functionof elbow flexion and forearm rotation difference.Results: All children with follow-up time of9-24months, children during thefollow-up period all healing, counterpoint good to the line of fracture, the fractures healedwell.5-6weeks to dismantle gypsum, disconnect the Kirschner wire, needle tract infectionsulna nonunion, delayed union or osteomyelitis. Forearm rotation postoperatively improvedsignificantly compared with the preoperative contralateral slightly different. Conclusion: The main reasons of resulting in old Monteggia’s fracture are misseddiagnosis and early incorrect treatment. The reduction of radial head is crucial, it is alsoimportant to improve curative effect that early functional exercise after operation wasmandatory for good operative results.
