Objective:To describe the incidence and to discuss the risk factors of premature birth in rural areas of Liuyang, Which to provide a basis for at-risk premature screening, monitoring and prevention work.Methods:Using both of the Prospective cohort study method and Cluster sampling methods, we collected our subjects from14villages and towns in Liuyang, Hunan province. The total number of villages and towns in Liuyang are37.Before entered in our cohort, all of them had established health manual during January2010to December2011.We then followed up the early, middle and late stages of pregnancy until delivery, and collected medical records and maternal health care manual of this cohort as our data materials. We carried on statistical description to analyze the main basic situation of our study objects. Adopting (X±S) to show continuous variables, while classification variables using proportion(%) and rate(%) to mean. We then calculated the incidence rate of premature birth in total population. We explored the main influence factors of premature delivery by using χ2test and unconditional Logistic regression analysis for single factor and multivariate analysis respectively, based on which to explore the main influence factors of preterm birth.Results:1. A total number of6268pregnant women were entered in our cohort, of which265cases were diagnosed as preterm birth. Incidence of preterm birth was4.22%.2. The newborn consists of3302boys and2874girls, accounting for53.5%and46.5%respectively. We obtained6204singleton pregnancy cases and64twin pregnancy and multiple pregnancy cases.3. Our results showed that determinants of premature birth were as follows:placental abruption、abnormal uterine and malformation、 pregnancy complications and Male fetus; the main determinants of SPTB were abnormal uterine and malformation and male fetus; and the protective factors of preterm birth and SPTB were prenatal examination more frequently and single birth.Conclusion:1. The preterm birth rate in Liuyang is4.22%.2. We should find those risk factors of preterm birth in time, such as placental abruption, abnormal uterine and malformation and Male fetuses.3. This investigation found that more frequencies of prenatal check and single pregnancy were the protective factors of premature birth. Regularly prenatal examination should be carried out from the beginning of pregnancy. Found that risk factors in the early time, prevention and treatment can reduce the rate of preterm birth.
