Objective:To understand schistosome infection status and liver and spleen abnormal situation of the professional fishermen in Yueyang County, East Dongting Lake Region. And to explore the concerned influencing factors, all that is to provide base for developing the scientific schistosomiasis Control Countermeasures in fishermen.Methods:A cross-sectional survey method was adopted in the study,275professional fishermen of two fishermen gathered Village in Yueyang County was sampling out as the research objects, and schistosomes pathogenic examination were carried out for all objects, and then calculate the infection rate of the survey population and Geo-EPG of patients; simultaneously, a questionnaire survey were carried out in the objects, include:general population characteristics, professional fishing habits, family economic status, and schistosomiasis treatment, chemotherapy, etc. Contingency table chi-square test(χ2)and multivariate unconditional logistic regression analysis were used to explore the related factors of schistosome infection of fishermen; and then,129fishermen were sampling out, in accordance with the age structure stratified sampling of the survey population, as the objects for schistomiasis detection with B-ultrasound. Collect the B-ultrasound data of liver and spleen; calculate schistosomal hepatomegaly rate, splenomegaly rate, hepatic fibrosis rate, portal expansion rate and liver and spleen abnormal rate of the population, and then multivariate unconditional logistic regression analysis were used to explore the influencing factors of liver and spleen damage. Statistical software EXCEL2003and SPSS13.0were used in the study.Results:1. The average age of survey population is44.47±9.77, and the overall age structure of the population is too large and the overall cultural level is too low and the economy status of the population is not strong. The total infection rate of the fishermen population is40.36%(111/275), the geometric mean of EPG of patients is17.4±4.4; the infection rate of fishermen in Matang was43.75%(35/80); and38.97%(76/195) in fishermen of Lujiao.2. The results by univariate analysis show that:age, sex, annual per capita income, fishing work experience, schistosomiasis treatment history, number of schistosomiasis treatments, chemotherapy history in2008are the meaningful impact factors of schistosome infection in fishermen population(P<0.05).3. The results by multivariate unconditional Logistic regression analysis showed that the per capita annual income, fishing work experience, age, gender, number of schistosomiasis treatment and chemotherapy history in2008were main influence factors of schistosome infection in fishermen, OR value respectively was2.850(95%CI:1.797to4.519),1.740(95%CI:1.106to2.754),0.648(95%CI:0.470~0.895),0.445(95%CI:0.238~0.832),0.381(95%CI:0.240~0.606),0.331(95%CI:0.174~0.632).4. B ultrasound data showed the hepatomegaly rate was34.9%(45/129), splenomegaly rate was20.2%(26/129), the portal expansion rate was10.1%(13/129), hepatic fibrosis was58.9%(76/129) and total liver and spleen abnormalities rate was72.1%(93/129) in the sampled fishermen population.5. the results of B-ultrasound indicators by multivariate unconditional logistic regression analysis showed that annual household net income, the fishing work experience were the main risk factors for hepatic fibrosis of fishermen, OR respectively is OR2.13(95%CI:1.01to4.56),2.82(95%CI:1.37to5.82); annual household net income was the main risk factor of hepatomegaly of fishermen, OR was1.72(95%CI:1.03to2.87); and age was the meaningful factor of splenomegalyage of fishermen, OR was1.66(95%CI:1.05to2.63); fishing work experience was the main risk factor of portal expansion of fishermen, OR was6.64(95%CI:1.07to41.32); and female was protective factors for hepatic fibrosis, hepatomegaly, of fishermen. OR was0.13(95%CI:0.05-0.31),0.25(95%CI:0.10-0.61) respectively.Conclusion:1. The schistosome infection rate of professional fishermen in Yueyang County was as high as40.36%, the liver and spleen abnormal rate was up to72.09%in the sampled fisherman population, all that showed the schistosome infection rate and the damage of liver and spleen were still severe in fishermen of Yueyang County, Eastern Donting Lake Region, and need to further strengthen measures to monitor and control.2,The per capita annual income and fishing work experience were the major risk factor for fishermen schistosome infection; higher per capita income and fishing work experience, higher risk of schistosome infection in fishermen. Addition, age, gender, number of schistosomiasis treatment, chemotherapy history in2008also were the main influence factors of fishermen schistosome infection, the female fishermen with greater age, higher number of schistosomiasis treatments, and get chemotherapy in2008were in lower risk of schistosome infection.3. Annual household net income, the fishing work experience were the main risk factors for hepatic fibrosis of fishermen; annual household net income was the main risk factor of hepatomegaly of fishermen; and age was the meaningful factor of splenomegalyage of fishermen; fishing work experience was the main risk factor of portal expansion of fishermen; and female was protective factor for hepatic fibrosis, hepatomegaly of fishermen.
