The Study On The Equalization Of Basic Health Services In Chengdu On The Background Of Balancing Urban And Rural Development

People take more attention to the level of the equalization of basic medical health services, with continuous deep of medical and health care reform in our country. The equalization of basic medical health services not only affect the health condition of citizens but also improve the sustained medical system development and harmonious society.It is vital to promote the social equity and social justness. Domestic scholars study the question of the equalization of basic medical health services mainly through macro view of inter-provincial and inter-urban and rural in the nationwide. Some scholars analyze the problem based on the perspective of the regional and provincial levels. However, most researchs lack of in-depth analysis in a typical area which implement a common basic health policy.Chengdu as a urban and rural comprehensive reform pilot area, aimed at changing the urban-rural dual economic structure and exploring the urban and rural development.The equalization of basic health services in Chengdu is advancing progressively as the development of the reformation in the health system.This paper,based on the perspectives of regional and urban-rural,in-depth analyzed the level of the equalization of masic medical health service in Chengdu to enrich the research.Firstly.this paper discusses the connotation of the equality of basic medical health, the relevant academic and practical studies in external countries, and also expounds the theoretical basis.Then the paper analyzes Satisfaction, supply and demand of basic medical health services in the course of rural-urban integration in Chengdu.The results show that the main influence factors of non-equal basic medical services in Chengdu is shortage in the ability of basic medical services, imperfect mechanism of supervision and evaluation, the difference of information management of urban-rural basic medical services.Secondly,based on the definition of the connotation of basic medical health service, we construct an index system for evaluating the level of the equalization of basic medical health service. Then based on the data of2010,the paper determines the index weight with entropy weight method, and applies the TOPSIS method for the research of evaluating the level of equalization of basic medical health service in Chengdu. The analys is results indicate that technology staffing and facilities configuration are the main factor affecting the level of equalization;the equalization level of Chengdu shows the significant characteristics of circle layer; the obvious gap between the areas of different circle layer and the center of the city is significantly higher than outlying areas;the level of equalization in the same circle layer regions are close.Thirdly,this paper uses the GE index and Gini coefficient to calculate the Per capita allocation situation for Basic medical health service resource between urban and rural areas in Chengdu. Results show that, urban-rural gap has been the first factor contributing to the general gap,rural areas’s interregional gap has been the second factor, after that is the urban areas’s interregional gap.The the general gap of the second circle layer regions of Chengdu is less than that of the third circle layer regions.The urban-rural gap of allocation situation of facilities is less than that of human resources allocation obviously. The reasons contributing to changing the urban-rural gap of allocation situation for basic medical health service resource are analyzed in depths.Finally,propose a countermeasures which is beneficial to the equalization of basic medical services in Chengdu,on the basis of foregoing analysis. Promote the basic medical services in Chengdu through taking some measures such as perfecting supply decision mechanism of urban-rural basic medical services,breaking through the restriction of career and identity and urban-rural limitation, establishing a unified urban and rural guarantee system of medical service, establishing a reasonable fiscal transfer payment system, strengthening the construction of talent team in basic medical and health institutions,and perfecting the system of urban-rural basic medical services.
