Objective:After generalizing the theories of employing traditional Chinesemedicine(TCM) external application therapy after fracture,evaluating the functionand curative effect of the therapy in promoting fracture healing process,and basedon the establishment of rabbit fracture model,the thesis observes the change inbroken bone edge and the formation of bone callus shadow after applying the Jiegupaste through medical imaging;the fracture healing and the expression of BMP-7histologically and immunohistochemically;also explores the acting mechanism ofTujia Jiegu paste in promoting the fracture healing at the level of molecule.Method:(1)Theoretical study:Through consulting substantial ancient andmodern literature,the thesis systematically organizes fracture the origin,etiologyand pathogenesis,diagnosis and treatment;expounds the theoretical and practicalvalue of fracture application method.(2)Experimental study:48adult healthy big-ear white Japanese rabbits areselected to build3mm bone defect of the left radius model.Randomly divided intothe Jiegu paste experimental group (hereinafter referred to as the experimentalgroup)and the model control group (hereinafter referred to as the control group),24rabbits in each group.1day after the model and the incision dried,theexperimental group are applied Tujia Jiegu paste and bandage fix on the principle ofsmall splint fixed treatment of fracture;the control group are not applied but bandagefix with the identical splint.The experimental rabbits are killed with air embolismmethod at the end of the2nd week,4th week and6th week respectively(each group8a time) after DR radiography.And then conduct bone specimen (paraffinembedded) biopsy,HE conventional staining,immunohistochemical detection,toexplore the expression and change of BMP-7in fracture healing at molecule level.(3)All the data are statistically dealt with SPSS12.0software,said with x±s,and contrasted t-test with two sample means.Result:(1)The theoretical discussion of the application method:Application method,with a long history,convenient use,noticeable curative effect,economy and security,is an important part of external treatment of TCM and thecommon way to externally treat closed fracture.(2)Digital radiography(DR):Some relatively clear fracture lines can be seen inthe two groups by the end of the2nd week.The experimental group has callusformation,and only periosteum reaction appeared in the control group.Higher callus shadow density is also detected in the experimental group compared with the controlgroup.By the4th weekend,fracture lines are close to disappear and the externalcallus bridging fracture end area is gradually reducing; marrow cavity notcompletely unblocked fully in the experimental group.In the control group visiblecallus,but marrow cavity not the unobstructed.And the callus shadow density andthe callus area in the experimental group are better than that in the control group.Bythe end of the6th week,faster growing callus,osseous connection at fracture ends,complete disappearance of fracture lines and fully unobstructed marrow cavity existin the experimental group;the control group can still see blurred fracture lines,obvious callus,reduced bridging fracture ends,and the area is smaller,unobstructednot marrow cavity.All the experimental groups are superior to the control groups incallus formation quality,shadow density and callus quality grading.There is asignificant difference between them(P<0.01).(3)Hematoxylin-eosin staining(HE staining):Through light microscopicobservation,in the experimental group,there has considerable fibrous tissueconnection in the fracture area,which is earlier than in the control group,calciumsalt group deposition,minor trabecular bone and cartilage composition in some areaby the2nd weekend.By the end of the4th week,fibrous callus formation in fractureends region,OB number and callus content in the experimental group are more thanthose in the control group.And lamellar bone formed,less cartilage existed,marrowcavity basically rechanneled in the experimental group.By the end of the6th week,much more callus and lamellar bone formed,marrow cavity channeled and osseousconnection time became earlier in the experimental group,visible trabecular boneand little cartilage composition lay in the control group.(4)Immunohistochemical detection:At each time phase point,the measuredBMP-7MOD values in these two groups are conducted sample mean t test,showedremarkable difference(P<0.01);the volume of BMP-7at each time phase point inthe experimental group obviously higher than that in the control group,andapparently prolonged the expressing time.Conclusion:Fracture external application method,with a long history,convenient to apply,is the common way to externally treat closed fracture.Theapplication method for curing fracture with Tujia minority medicine Jiegu paste canimprove the fracture blood circulation greatly,promote hematoma absorption andcalcium deposition.Enhance and extend the volume of BMP-7at each time phase point to promote fracture callus formation,accelerate callus growth and osseoushealing,improve callus quality.And thus,to promote fracture healing and shortenthe healing time....
