Objective: To investigate the response of inhaled glucecotticoid in positive sputumeosinophils of patients with COPD. To analyze the correlation between fractional exhalednitric oxide (FENO) and eosnophils in COPD patients with exacerbations(AECOPD),toexplore whether FENO can take the place of eosinophils become is a effective indicator formonitoring the eosinophilic inflammation in COPD patients with exacerbations and whether itis a good indicator for evaluating glucocorticoid therapy of AECOPD and guide Clinicaltreating.Methods: Select47hospitalized AECOPD patients, According to phlegm eosinophilic cellsroportion and whether hormone therapy,These AECOPD patients can be divided into sputumeosinophils positive and hormone therapy group (group A), sputum eosinophils negative andhormone therapy group (group B), sputum eosinophils positive and wiithout hormonetreatment group (group C), sputum eosinophils negative and without hormone treatmentgroup (group D).A、 B group are treated with routine cure and nebulized budesonid,2mg/time,2times a day,7days in a row. C, D group just given regular treatment. All of thepatients are measured FENO、sputum eosinophilic cell count,、blood gas analysis,and CATscore on the first day(or the second day) and treatment after8days.Results: FENO, EOS levels in group A were lower than in group C after treatment, thedifference has statistical significance(P<0.05). FENO after treatment was reduced in group C,the difference was statistically significan(tP<0.05). No obvious difference was found betweenbefore and after treatment group C of the EOS(P>0.05). FENO, EOS levels of beforetreatment. in group B and group D have no significant difference compared with that o f aftertreatment(P>0.05). There was a significant positive correlation between FENO and thepercentage of sputum eosinophils in Patients with AECOPD, both before(r=0.702, P<0.05)and after the treatmen(tr=0.804,P<0.05).the differences of oxygen partial pressure and CATscore before treatment among A、B、C、D groups were no significant (P>0.05). oxygen partial pressure, the CAT score after treatment in every group were improved than beforetreatment (P <0.05), A group improved more significantly (P <0.05).Conclusion: sputum eosinophils positive of patients with AECOPD have better responseto inhaled glucecotticoid. FENO is a effective indicator for monitoring the eosinophilicinflammation in COPD patients with exacerbations,it is a good indicator for evaluatingglucocorticoid therapy of AECOPD,It is worth clinical application....
