Objective:To compare the clinical effect between periodontal flap surgery and repeated periodontal scaling and root planing (scaling and root planing, SRP) of chronic periodontitis.Methods:Samples of117single-rooted teeth, which still had deep pockets from5mm to7mm at6weeks after initial therapy, were selected. The right and left sides of the same patient teeth were randomly divided into repeated SRP group and flap surgery group. The repeated SRP group consists of30patient,58teeth; the flap surgery group consists of30patients,59teeth. The repeated SRP group of patients receives scaling and root planing with local anesthesia on the troubled teeth employing Sately ultrasound therapy machines and Gracey curette; the flap surgery group received modified Widman flap surgery under local anesthesia. Patients returned visit after3and6months for professional periodontal examination. Record for the experimental therapy noted as: bleeding on probing (bleeding on probing, BOP), probing depth (probing depth PD), clinical attachment level (clinical attachment level, CAL)and gingival recession (the gingival recession GR).Results:BOP%,PD,GR,CAL values at baseline in second subgingival scaling group and surgery group are respectively91.45%、(5.47±0.93) mm、(5.89±1.15) mm、(0.34±0.47) mm and93.27%、(5.91±1.05)mm、(6.07±1.33) mm、(0.32±0.49) mm. And all indexes in two group s at6m after therapy are respectively16.92%、(2.82±0.74) mm、(5.05±1.02) mm、(2.02±0.60) mm and11.60%、(2.54±0.49) mm、4.90±0.83) mm、(2.35±0.67) mm, which are significantly improved than those at baseline in both groups (P<0.01). However, BOP%, PD and CAL values at final examination show no significant differences between two groups (P>0.05). Both treatments caused gingival recession after surgery, but gingival recession of the flap surgery group is significantly higher than that of those received second subgingival scaling therapy (P<0.05).
