ObjectiveTo explore the demand and utilization of health service in Hunan non-governmentcollege students and to research the present situation of the medical security andinfluencing factors. And to find the problems about student medical security innon-government colleges, in order to provide foundations when we make the healthcaresystems and measures which can promote the non-government college students health.MethodRandom cluster sampling was used among2438students, randomly in45classes,three class of each grade at every college, four non-government colleges in Hunan,including two undergraduate colleges and two junior school, using the health serviceutilization and medical security questionnaire in non-government colleges to researchthe health service need of Hunan non-government colleges and universities students andresearching the present situation of the medical security and influencing factors,class-based.Results1. The health service demand and its influencing factors in non-government college:The non-government colleges student’s two-week prevalence and the six-month chronicdisease prevalence were15.92%and2.73%respectively. There was no statisticallysignificant difference of two-week prevalence among students of different familyeconomic conditions, among students of different non-government colleges, betweenundergraduate course and specialized subject students in non-government colleges (P>0.05), freshmen was higher than other students(P<0.05), girls was higher thanboys(P=0.006), the town students were higher than the countryside’s (P=0.008), badhabits students were higher than good ones (P=0.000). The physical exercises had affecton two-week prevalence. The more the students did, the lower the two-week prevalencewas. Never did any kind of physical exercise be the most one. There were only14.55%of the students who often did physical exercise,35.05%ones never and65.92%oneswith bad habits. Grade, academic levels, the students from countryside or town anddomestic economic situation had no effect on the the half-year chronic diseaseprevalence.2. The health service utilization and influencing factors in non-government college:during the non-government college, the rate of two-week visiting doctor was39.9%, therates of the hospital admission every year and non-hospitalization were4.23%and2.29%respectively. The rate of two-week visiting doctor in medical college was worsethan in non-medical college(P=0.008), special students was higher than undergraduatestudents(P=0.002), girls was higher than boys(P=0.043), the town was higher than thecountryside(P=0.030). Incomes affected the two-week visiting doctor rate(P=0.024), theworse the family incomes was, the lower the rate was. The special students andundergraduate students with the two-week prevalence chose the different medicalInstitutions as the first one to see a doctor. The undergraduate students went to schoolclinic and community health service center to see a doctor, and the rate was69.1%.There was no school clinic in two special schools, so the two-week prevalence chose theprivate clinic and community health service center as the first ones.The self-treatment rate of two-week was33.1%, the medical students of that higherthan the other department students(P=0.036), the students from the town higher thanfrom the countryside(P=0.004). Grade, sex, income, habit, physical exercise and so onwere irrelevant to the self-treatment rate of two-week(P>0.05).29.6%students oftwo-week had no treatment, because the most important reason was economic hardship,then the second one was no treatment with the mild illness, and another important onewas the bad quality of hospital services and medical technology. The students also werenot satisfied with the school medical service. 3. The current situation of medical security in non-government college: The rate ofhealth insurance for urban residents and the rate of accident insurance were96.7%and94.4%. The awareness rates of those was very low, and the ones were59.7%and58.8%.There was no difference with the rate of health insurance for urban residents and therate of accident insurance in each school(P>0.05). But the awareness rate of theundergraduate students was higher than the special students. The student thought thatschool was without any health insurance and measures and the health insurance andmeasures did not have any effect even though the school had that. The awareness rate ofhealth insurance and measures in school was only42.3%. Health insurance for urbanresidents and accident insurance to the non-government college students weremandatory, but the without rate was more than30%. Because the student didn’t knownthe health insurance and measures, the cost of insurance was very high, thereimbursement rate was too low and even with few effect. Claims trouble was one of themain reasons why the students didn’t want to have the insurance.Only8.7%students with the insurance obtained the medical expense reimbursement,4.3%students reimbursed hospitalization,4.3%students reimbursed the office visit, and1.7%students reimbursed both.Conclusion1. The visiting doctor rate and the hospital admission rate in the Hunan provincenon-government college students who are ill are low, so the non-government collegeshould strengthen the students’ consciousness of medical care and guide their rightbehavior of seeing a doctor.2. The Hunan province non-government college lack the prevention and health care,the health education and the medical facilities, should develop the medical insurancefinancing, build and improve the school clinic and the medical aid system, in order tomake the school clinic become the community service center.3. The Hunan province non-government college students have been incorporatedinto the national basic medical guarantee and build a variety of health-care system. Butthe students’ consciousness of insurance is not strong, the awareness rate of the health insurance and measures is too low, the students are mandatory by the school and therate of suing also is low. So according to kinds of methods, the school should developthe rate of the college students by themselves.
