Objective:The aim of our study is to investigate the survival and prognostic factors in Chinese Hodgkin’s lymphoma patients.Methods:According to the classification of WHO criteria,415previously untreated patients with histologically confirmed Hodgkin’s lymphoma in Cancer hospital, Chinese Academy of Medical Sciences from February1999to February2011were analysed for treatment response rates, long-term survival, as well as prognostic factors. Meanwhile, treatment response rates, long-term survival and prognostic factors were analysed for patients with early stage favorable HL, early stage unfavorable HL and advanced HL according to German Hodgkin Study Group(GHSG) criteria.ResultsrFor the whole group, the rates of complete remission(CR), partial remission and disease progression were89.4%,8.0%and2.7%. The CR rates for early stage(stage Ⅰ/Ⅱ) and advanced stage patients (stage Ⅲ/Ⅳ) were92.9%and80.7%. At67months(4-166months) median follow-up,62relapse events and35deaths were reported. Disease free survival, progression free survival(PFS) and overall survival(OS) at10years were80.0%,82.4%and91.6%.10-year PFS and10-year OS patients for early stage were89.5%and95.9%, and for advanced stage patients were64.7%and80.7%. Univariate analysis showed advanced stage, B symptoms, elevated serum β2-microglobulin and chemotherapy regimen MOPP were related to lower CR rate and poorer PFS and OS. Elevated serum lactate dehydrogenase, nodular lymphocyte-predominant Hodgkin lymphoma and CD20-positive were related to poorer PFS. ECOG2-3and age≥45were associated with poorer OS. Multivariate analysis showed that advanced stage and CD20-positive as independent unfavorable prognostic factors influenced PFS, and age≥45, advanced stage, elevated serum β2-microglobulin and chemotherapy regimen MOPP as independent unfavorable prognostic factors influenced OS. Fifty-three (14.2%) had early stage favorable,173(46.5%) had early stage favorable and146(39.2%) had advanced stage HL of the372patients that can be classified according to GHSG criteria. The CR rates,10-year PFS and10-year OS were significantly better for early stage favorable HL (94.3%,90.6%and96.2%) and early stage unfavorable HL (93.6%,89.6%and97.1%) compared with advanced stage HL(80.8%,67.8%and82.2%).Conclusion:Patients with HL treated in our hospital have excellent prognosis.OS at10years for patients with early stage favorable, early stage favorable and advanced stage HL were96.2%,97.1%and82.2%, which was consistent with those in Western countries.
