Y-sheaped Osteotomy For Developmental Coax Vara Of Children

Objective: To investigate the method and therapeutic effect of Y-shaped osteotomyfor treatment of developmental coxa vara. Methods: Since January2008to October2011, treatment for children’s developmental coxa vara10cases (14hips),male4cases(5hips),female6cases (9hips),age5to12years old,an average of7.8years old.all thechildren had obviously lameness,different degrees hip abduction,adduction and rotaionlimited.All cases take the anteroposterior pelvic X-ray,and measuring collodiaphysialangle range of46to110°,an average of87°,HE angle range of36to93°,an averageof57°,and the ATD value range of-25to6mm,an average of-3mm.All cases are usedthe subtrochanteric Y-shaped osteotomy,and the outreach osteotomy plate to internalfixation. Results: In this group all patients were followed up, it for aminimum of14months up to40months, an average of follow-up is18months. According to LiuJianDe ’s assessment standards,there were5hips result is excellent,8hips result is good,1hip result is fail, the percentage of result which excellent and good is92.8%.Postoperative week review the X-ray shows,the vertical epiphyseal plate of proximalfemur was returned to the horizontal. Rehabilitate the structure of hip,thecollodiaphysial angle range of130to135°,an average of137°,HE angle range of23to35°,an average of27°,and the ATD value range of3to22mm,an average of14mm.All the children had no recurrence of coax vara, except1case was occurred in the30months after the surgery. Conclusion:The Y-shaped osteotomy and using outreachosteotomy plate to internal fixation can give full orthopedic and strongly fixation. Thesurgery is increasing the length of the affected limb, and after surgery is no need toplaster immobilization. Patient can do exercise after surgery, even can ambulation withfree weight-bearing soon. It is a satisfactory clinical outcome. Therefore, it is an effective method for the treatment of developmental coxa vara.
