Objective Comparative analysis of the effects of Salter osteotomy and Pemberton osteotomy on femoral head necrosis rate and prognosis outcome in children with childhood developmental dislocation of the hip,in order to find the best surgical method,it provides a theoretical basis for the selection of optimal surgical treatment for childhood developmental hip dislocation.Methods Ninety children with childhood developmental dislocation of the hip were treated in our hospital from June 2009 to January 2013.They were divided into two groups according to the treatment plan adopted for osteotomy.Sixty-three children in the Salter group were treated with Salter osteotomy.37 children in the Pemberton group were treated with Pemberton osteotomy.The treatment effects,X-ray follow-up,improvement of postoperative imaging parameters,and femoral head necrosis rate were compared between the two groups.Results There was no significant difference in the rate of treatment between the Salter group and the Pemberton group(P>0.05).At the last follow-up,the radiographic parameters such as CE angle,Bony acetabular index and Reimers instability index in the Salter group and the Pemberton group were significantly improved(P<0.05).Compared with Salter group,the bone acetabular index improved more in children with Pemberton(P<0.05),but there was no significant difference between the two groups in CE angle and Reimers instability index(P>0.05).The rate of femoral head necrosis was significantly lower in the Salter group than in the Pemberton group(P<0.05).The femoral head necrosis rate was also different in children of different ages.The necrosis rate of femoral head in children aged 4 years or younger was significantly lower than that in children aged >4 years(P<0.05).Conclusion Salter osteotomy and Pemberton osteotomy have good curative effects in the treatment of childhood developmental hip dislocation.Although the bony acetabular index of children after Pemberton osteotomy can be significantly improved,but Salter osteotomy can be more effective in improving the prognosis of children,significantly reducing the rate of femoral head necrosis.
