Objective To explore the clinical effect of selective laminectomy and internalfixation for degenerative lumbar spina1stenosis.Methods:59cases with degenerative lumbar spinal stenosiswere reviewedretrospective1y.According to Hansraj’s c1assifcation of lumbar spinal stenosis andlevels responsive to symptoms,14cases with c1assic degenerative lumbar spinalstenosis were treated with selective laminectomy alone.21of45cases with complicateddegenerative lumbar spinal stenosis were treated with selective laminectomydecompression and fusion,while another24cases underwent complete1aminectomyand fusion.The c1inica1outcome(including neurofunction and self-assessment) atpreoperation and follow-up was analyzed by Japanese Orthopaedic Association(JOA)score,the results were put into statistical analysis.Results:Allpatients were followed up for an average of32.7months (range,8months to36months).The improve rate of JOAscoreat final follow-up for each groupwas58.2%±34.0%,61.7%±23.6%and56.4%±26.8%respectively,with excellent togood rate for each group of72.7%,69.2%and71.4respectively.Which showed nostatisticaldifference among three groups(P<0.05).All patients were satisfied withsurgical outcomes.Conclusion:Selective laminectomy is reliable fordegenerativelumbar spinal stenosis.Acknowledging of surgical indication and range of laminectomycan ensurea good outcome either by means of selective laminectomy alone or completelaminectomy and fusion....
